摘要
目的探究基于3D导航技术的经导管肝动脉化疗栓塞(TACE)在肝癌术后新发肝脏影像报告和数据系统(LI-RADS)LR-5小结节治疗中的应用价值。方法对17例肝癌外科术后患者中,增强MRI检测出的共计51枚LI-RADS LR-5小结节,行Embolization Guidance 3D导航下的超选择TACE治疗。采用组内相关系数(ICC)评价TACE术中小结节检出率,评价TACE术后小结节碘油沉积,回顾性搜集术后并发症。结果51枚LI-RADS LR-5小结节于TACE术中均实现完全检出(ICC=1.00)。TACE术后病灶区域碘油Ⅰ型沉积51个,完全栓塞成功率100%(51/51)。术后并发症包括低热3例、肝区疼痛5例,均为轻度不良反应。结论3D导航技术可应用于肝癌术后新发LI-RADS LR-5小结节的TACE治疗,提高栓塞成功率并降低患者不良反应。
Objective To study the application value of TACE operation based on 3 D navigation technology in the treatment of new LI-RADS LR-5 small nodules after liver cancer surgery.Methods A total of 51 LI-RADS LR-5 small nodules detected by enhanced MRI in 17 patients who were treated with Embolization Guidance 3 D navigation under the superselective TACE treatment after liver cancer surgery.The intra-group correlation coefficient(ICC)was used to evaluate the detection rate of small nodules during TACE,to evaluate the deposition of lipiodol in small nodules after TACE,and to collect postoperative complications retrospectively.Results 51 LI-RADS LR-5 small nodules were completely detected during TACE(ICC=1.00).There were 51 lipiodol type I deposited in the lesion area after TACE,and the success rate of complete embolization was 100%(51/51).Postoperative complications included 3 cases of low fever and 5 cases of liver pain,all of which were mild side effects.Conclusion 3 D navigation technology can be applied to TACE treatment of new LI-RADS LR-5 small nodules after liver cancer surgery,which can improve the success rate of embolization and reduce the adverse reactions of patients.
