经胆道镜钬激光治疗术后残留复杂胆道结石25例

Choledochoscopic holmium laser for 25 cases of postoperative residual complex biliary calculi

目的探讨胆道镜下用钬激光经瘘道碎石治疗术后胆道复杂残留结石的临床价值。方法2016年1月至2019年12月广州医科大学附属中医医院收治复杂肝内外胆管结石(不含胆囊)患者25例,男性11例,女性14例,年龄(58.0±20.5)岁,经术后瘘道,在胆道镜下利用钬激光多次碎石手术,回顾分析入组患者手术治疗临床资料。结果25例患者均能取干净结石,恢复胆道通畅,随诊1年无复发,无胆道狭窄,无严重手术并发症。结论多次经瘘道用胆道镜钬激光治疗复杂残留结石是一种高效、安全手术方法。

Objective To investigate the clinical value of choledochoscopic holmium laser for postoperative residual complex biliary calculi.Methods Twenty-five(58.0±20.5)years old patients with postoperative residual complex biliary calculi were treated by choledochoscopic holmium laser lithotripsy several times at The Affiliated TCM Hospital of Guangzhou Medical University from January,2016 to December,2019,including 11 males and 14 females.Their clinical data were retrospectively analyzed.Results The stones were removed successfully in all the 25 patients,and their biliary tracts were unobstructed.After 1 year's follow-up,there were no recurrence,no biliary stricture,and no serious complications.Conclusion It is an effective and safe operation to treat postoperative residual complex biliary calculi with holmium laser under choledochoscope through fistula for several times.
