Q开关532 nm激光联合超脉冲CO_(2)激光治疗脂溢性角化病疗效观察

Therapeutic Effect of 532nm Laser with Q Switch Combined with Superpulse CO_(2) Laser on Seborrheic Keratosis

目的:探讨Q开关532nm激光联合超脉冲CO_(2)激光治疗脂溢性角化病的疗效。方法:选取2018年6月-2019年6月笔者医院和唐山市协和医院收治的84例脂溢性角化病患者为研究对象,以随机数字表法将其分为治疗组和对照组,每组42例。对照组采用超脉冲CO_(2)激光治疗,治疗组采用Q开关532nm激光联合超脉冲CO_(2)激光治疗,两组连续治疗3个月。比较两组治疗前、治疗3个月后的生活质量,观察两组治疗3个月后的临床疗效、修复差异及局部色素改变情况。结果:治疗3个月后,治疗组总有效率、疼痛分级0级者所占比例、色素无变化的发生率分别为95.24%、83.33%、88.10%,高于对照组(76.19%、57.14%、42.86%),两组比较具有统计学意义(P<0.05);治疗组创面愈合时间短于对照组(P<0.05)。治疗3个月后,治疗组疼痛分级I级者所占比例及色素减退、色素沉着发生率为16.67%、0.00%、11.90%,低于对照组(35.71%、38.10%、54.76%),两组比较具有统计学意义(P<0.05)。治疗3个月后,两组衣物选择、症状与躯体感受、日常活动、心理感受评分低于治疗前,治疗组低于对照组,两组具有统计学意义(P<0.05)。结论:Q开关532nm激光联合超脉冲CO_(2)激光治疗脂溢性角化病疗效显著,可减轻脂溢性角化病患者的疼痛程度,促进创面愈合,同时减少色素减退、色素沉着情况,并可提高患者生活质量。

Objective To analyze the therapeutic effect of 532nm laser with Q-switch combined with superpulse CO_(2) laser on seborrheic keratosis.Methods 84 patients with seborrheic keratosis admitted to our hospital and Tangshan Union Hospital from June 2018 to June 2019 were selected as the research objects,and they were divided into the treatment group and the control group by random number table,with 42 cases in each group.The control group received superpulse CO_(2) laser treatment,while the treatment group received 532nm laser with Q switch combined with superpulse CO_(2) laser treatment.The two groups were treated for 3 months.The quality of life of the two groups before and after 3 months of treatment was compared,and the clinical efficacy,repair difference and local pigment changes of the two groups after 3 months of treatment were observed.Results After 3 months of treatment,the total effective rate,the proportion of pain grade 0 and the incidence of pigment unchanged in the treatment group were 95.24%,83.33%and 88.10%,which were higher than those in the control group(76.19%,42.86%,57.14%,P<0.05),wound healing time in the treatment group was shorter than that in the control group(P<0.05).After 3 months of treatment,the proportion of patients with pain grade I and the incidence of hypopigmentation and pigmentation in the treatment group were 16.67%,0.00%and 11.90%,which were lower than those in the control group(35.71%,38.10%,54.76%,P<0.05).After 3 months of treatment,the scores of clothing selection,symptoms and somatic feelings,daily activities and psychological feelings in the two groups were lower than those before treatment,and those in the treatment group were lower than those in the control group(P<0.05).Conclusion The treatment of seborrheic keratosis with 532 nm laser with Q switch combined with superpulse CO_(2) laser was effective,which can reduce the pain of seborrheic keratosis patients,promote wound healing,reduce hypopigmentation and pigmentation,and improve the quality of life of patients.
