固元活血涤肾汤联合西药治疗慢性肾脏病3期临床研究

Clinical Study on Guyuan Huoxue Dishen Tang Combined with Western Medicine for Stage Ⅲ Chronic Kidney Disease

目的:观察固元活血涤肾汤联合西药治疗慢性肾脏病3期的临床疗效。方法:选取100例慢性肾脏病3期患者,随机分为治疗组和对照组各50例。对照组给予常规西药治疗,治疗组在对照组基础上给予固元活血涤肾汤治疗,2组均连续治疗2个月。比较2组临床疗效及不良反应发生率;观察2组治疗前后中医证候积分、血肌酐(SCr)、24 h尿蛋白定量、尿素氮(BUN)、白细胞介素-6 (IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C-反应蛋白(hs-CRP)水平的变化。结果:治疗组总有效率为86.00%,高于对照组的68.00%(P<0.05)。治疗后,2组腰膝酸软、面色晦暗、神疲乏力、头昏失眠积分均较治疗前降低(P<0.05),治疗组上述4项中医证候积分均低于对照组(P<0.05)。治疗后,2组SCr、24 h尿蛋白定量、BUN水平均较治疗前降低(P<0.05),治疗组SCr、24 h尿蛋白定量、BUN水平均低于对照组(P<0.05)。治疗后,2组血清IL-6、TNF-α、hs-CRP水平均较治疗前降低(P<0.05),治疗组血清IL-6、TNF-α、hs-CRP水平均低于对照组(P<0.05)。治疗组不良反应发生率为8.00%,对照组不良反应发生率为6.00%,2组比较,差异无统计学意义(P>0.05)。结论:固元活血涤肾汤联合西药治疗慢性肾脏病3期,可缓解患者的临床症状,改善肾功能,减轻炎症反应,安全性好。

Objectives:To observe the clinical effect of Guyuan Huoxue Dishen tang combined with western medicine for stage Ⅲ chronic kidney disease. Methods: A total of 100 cases of patients with stage Ⅲ chronic kidney disease were randomly divided into the treatment group and the control group,50 cases in each group. The control group was treated with routine western medicine,and the treatment group was additionally treated with Guyuan Huoxue Dishen tang based on the treatment of the control group. Both groups were continuously treated for two months. The clinical effect and incidence of adverse reactions in the two groups were compared. Before and after treatment,the change of Chinese medicine syndrome scores and levels of serum creatinine(SCr),24-hour urine protein,blood urea nitrogen(BUN),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α) and high-sensitivity C-reactive protein(hs-CRP) in the two groups were observed. Results:The total effective rate was 86.00% in the treatment group, higher than that of 68.00% in the control group(P<0.05). After treatment,the scores of soreness and weakness of waist and knees,complexion darkness,fatigue,and dizziness with insomnia in the two groups were decreased when compared with those before treatment(P<0.05), and the above four Chinese medicine syndrome scores in the treatment group were lower than those in the control group(P<0.05). After treatment,the levels of SCr,24-hour urine protein and BUN in the two groups were decreased when compared with those before treatment(P<0.05),and the levels of SCr,24-hour urine protein and BUN in the treatment group were lower than those in the control group(P<0.05). After treatment,the levels of IL-6,TNF-α and hs-CRP in serum in the two groups were decreased when compared with those before treatment(P<0.05), and the levels of IL-6, TNF-α and hs-CRP in serum in the treatment group were lower than those in the control group(P<0.05). The incidence of adverse reactions was8.00% in the treatment group, and 6.00% in the control group, there being no significance in the difference(P>0.05).Conclusion:Guyuan Huoxue Dishen tang combined with western medicine for stage Ⅲ chronic kidney disease can alleviate the clinical symptoms of patients,improve kidney function,and reduce inflammatory response,with good safety.
