促黄体生成素与促黄体生成素/促卵泡激素在痰湿型/非痰湿型多囊卵巢综合征患者中的特点及影响因素

Characteristics and influence factors of LH and LH/FSH in patients with phlegm-dampness type/non phlegm-dampness type polycystic ovary syndrome

目的:探讨促黄体生成素与促黄体生成素/促卵泡激素在痰湿型/非痰湿型多囊卵巢综合征患者中的特点及影响因素。方法:按照中医诊断标准将2018年1月-2019年12月湖北省孝感市中心医院收治的300例多囊卵巢综合征患者分为痰湿型组(185例)与非痰湿型组(115例)。收集两组患者的临床资料,比较两组患者的促黄体生成素水平及促黄体生成素/促卵泡激素值,分析促黄体生成素、促黄体生成素/促卵泡激素在不同中医证型患者中的影响因素。结果:痰湿型组的促黄体生成素水平、促黄体生成素/促卵泡激素值均显著低于非痰湿型组(P<0.05)。痰湿型组中,促黄体生成素与促卵泡激素、雌二醇、睾酮、脱氢表雄酮、雄烯二酮、高密度脂蛋白、脂蛋白a均呈显著正相关(P<0.05),与体质量、身体质量指数、腰围、臀围、黑棘皮评分、120 min血糖、胰岛素抵抗指数、空腹胰岛素、30 min胰岛素、60 min胰岛素、120 min胰岛素均呈显著负相关(P<0.05);促黄体生成素/促卵泡激素与雌二醇、孕酮、睾酮、雄烯二酮、高密度脂蛋白均呈显著正相关(P<0.05),与年龄、体质量、身体质量指数、腰围、臀围、黑棘皮评分、促卵泡激素、60 min血糖、120 min血糖、胰岛素抵抗指数、空腹胰岛素、载脂蛋白-B均呈显著负相关(P<0.05)。非痰湿型组中,促黄体生成素与促卵泡激素、雌二醇、孕酮、睾酮、雄烯二酮、180 min血糖均呈显著正相关(P<0.05);促黄体生成素/促卵泡激素与雌二醇、孕酮、睾酮、雄烯二酮均呈显著正相关(P<0.05),与催乳激素呈显著负相关(P<0.05)。结论:痰湿型与非痰湿型多囊卵巢综合征患者中促黄体生成素、促黄体生成素/促卵泡激素存在显著差异;体质量、身体质量指数、腰围、臀围及某些性激素指标、糖脂代谢指标为痰湿型多囊卵巢综合征患者中促黄体生成素、促黄体生成素/促卵泡激素的影响因素;雌二醇、孕酮、睾酮、雄烯二酮是非痰湿型多囊卵巢综合征患者中促黄体生成素、促黄体生成素/促卵泡激素的影响因素。

Objective:To explore the characteristics and influence factors of LH and LH/FSH in patients with phlegm-dampness type/non phlegm-dampness type PCOS.Methods:According to TCM diagnostic criteria,300 PCOS patients were divided into the phlegmdampness type group(185 cases)and non phlegm-dampness type group(115 cases)in our hospital from January 2018 to December 2019.The clinical data was collected in two groups.The LH level and LH/FSH value was compared between two groups.The influence factors of LH and LH/FSH in patients with different TCM syndromes were analyzed.Results:The LH level and LH/FSH value of phlegm-dampness type group was significantly lower.In the phlegm-dampness type group,LH was positively correlated with FSH,E2,T,DHEAS,AND,HDL and lipoprotein a(P<0.05).LH was negatively correlated with weight,BMI,waistline,hipline,acanthosis nigricans scores,120 min PG,IRI,FINS,30 min INS,60 min INS,120 min INS(P<0.05).LH/FSH was positively correlated with E2,P,T,AND,HDL(P<0.05),LH/FSH was negatively correlated with age,weight,BMI,waistline,hipline,acanthosis nigricans scores,FSH,60 min PG、120 min PG,IRI,FINS,APO-B,P<0.05.In the non phlegm-dampness type group,LH was positively correlated with FSH,E2,P,T,AND,180 min PG(P<0.05).LH/FSH was positively correlated with E2,P,T,AND(P<0.05),LH/FSH was negatively correlated with PRL,P<0.05.Conclusion:There were significant differences in LH and LH/FSH between phlegm-dampness type and non phlegm-dampness type PCOS patients.The weight,BMI,waistline,hipline and some sex hormone indexes,sugar fat metabolism indexes were the influence factors of LH and LH/FSH in patients with phlegm-dampness type PCOS.The E2,P,T,AND was the influence factors of LH and LH/FSH in patients with non phlegm-dampness type of PCOS.
