良性阵发性位置性眩晕的中医证候特征研究

Research on the Traditional Chinese Medicine Syndrome Characteristics of Benign Paroxysmal Positional Vertigo

目的探索良性阵发性位置性眩晕的中医证候特征,为其中医辨证和治疗提供参考。方法采用临床流行病学调查方法,收集82例良性阵发性位置性眩晕患者的基本信息、中医症状、舌相、脉象、中医证型等资料,应用描述性统计分析总结其主要症状、舌相、脉象特点、证素及中医证候分布特点。结果82例患者中有眩晕症状者53例(64.6%),眩晕为其最常见症状,其后依次为恶心(58.5%)、视物旋转(50.0%)、呕吐(45.1%)、行走不稳(42.7%)、头重如裹(42.7%)、胸闷(41.5%)、失眠(40.2%)、头晕(35.4%)等;舌质以舌淡胖、舌暗、舌红及舌淡为主,舌苔以苔白腻、苔薄白及少苔为主;脉象以脉弦滑、脉弦、脉滑及脉涩多见;中医证型以痰浊中阻最多(31.0%),其后依次为肝肾阴虚(24.4%)、痰瘀阻窍(19.5%)等,不同性别患者的中医证型分布差异无统计学意义,P>0.05,不同年龄段的中医证型分布差异有统计学意义,χ^(2)=63.717,P<0.05。结论良性阵发性位置性眩晕以中老年女性多见,以眩晕、恶心、视物旋转、呕吐、行走不稳、头重如裹、胸闷、失眠、头晕等为主要症状,中医证型以痰浊中阻、肝肾阴虚、痰瘀阻窍为主,不同性别患者的中医证候分布差异无统计学意义,不同年龄段患者的证候分布差异存在统计学意义。

Objective To explore the traditional Chinese medicine(TCM)syndrome characteristics of benign paroxysmal positional vertigo,and to provide reference for TCM syndrome differentiation and treatment.Methods 82 cases of benign paroxysmal positional vertigo were investigated by clinical epidemiology.The basic information,TCM symptoms,tongue,pulse and TCM syndrome types were collected.Descriptive statistical analysis was used to summarize the main symptoms,tongue signs,pulse characteristics,syndrome elements and TCM syndrome distribution characteristics.Results Among the 82 patients,53(64.6%)had vertigo was the most common symptom,followed by nausea(58.5%),visual rotation(50.0%),vomiting(45.1%),walking instability(42.7%),head weight(42.7%),chest tightness(41.5%),insomnia(40.2%),dizziness(35.4%);The tongue is mainly light fat,dark,red and light,and the coating on the tongue is mainly white and greasy,thin and white.Pulse condition is more common with pulse chord slip,pulse chord,pulse slip and pulse astringency.The most TCM syndromes were obstruction of phlegm(31.0%),followed by deficiency of liver and kidney yin(24.4%)and obstruction of phlegm and stasis(19.5%).There was no significant difference in the distribution of TCM syndromes in different genders(P>0.05),but there was significant difference in the distribution of TCM syndromes in different ages(χ^(2)=63.717,P<0.05).Conclusion The main symptoms of benign paroxysmal positional vertigo are vertigo,nausea,visual rotation,vomiting,unstable walking,heavy head,chest tightness,insomnia,dizziness,etc.TCM syndrome types are mainly phlegm turbidity,liver and kidney yin deficiency,phlegm and blood stasis obstructing orifices.There is no significant difference in TCM syndrome distribution among different gender groups,and the syndrome distribution of different age groups exists in the significant difference.
