护卵汤加减治疗小卵泡排卵患者对彩超监测指标的影响

Effects of Modified Huluan Decoction on color Doppler ultrasound monitoring indexes of patients with small follicle ovulation

目的观察在彩色超声多普勒(彩超)监测下护卵汤加减治疗小卵泡排卵患者临床疗效。方法将我院89例小卵泡排卵患者按不同治疗方式分为观察组(45例)与对照组(44例)两组。对照组采用克罗米芬治疗,观察组在对照组治疗基础上采用护卵汤加减治疗。比较两组患者月经第2天、第7~8天、卵泡后1~2d、排卵后7~10d雌二醇(E2)水平、出现排卵现象黄体生成素(LH)、排卵后7~10d孕酮(Prog)水平及妊娠情况,彩超监测排卵时子宫内膜的厚度、围排卵期宫颈评分。结果月经第2d,两组血清E2水平比较无显著差异(P>0.05)月经第7~8d、排卵后7~10d,观察组血清E2水平明显高于对照组,排卵后1~2d观察组血清E2水平低于对照组(P<0.05);两组出现排卵现象时LH水平、排卵后7~10d Prog水平比较无显著差异(P>0.05);观察组排卵时子宫内膜厚度、围排卵期宫颈评分均高于对照组(P<0.05);两组均观察3个周期妊娠率比较无显著差异(P>0.05)。结论护卵汤加减能显著改善小卵泡排卵患者体内性激素水平、子宫内膜厚度和宫颈评分,促进卵泡发育,提高妊娠率;彩超监测对于判断小卵泡排卵患者的疗效具有重要作用。

Objective To observe the effect of Modified Huluan Decoction on patients with small follicle ovulation under the monitoring of color Doppler ultrasound(color Doppler ultrasound).Methods A total of 89 patients with small follicle ovulation were divided into observation group(45 cases) and control group(44 cases) according to the treatment method.Control group was treated with clomiphene citrate,and in observation group was treated with Modified Huluan Decoction on the basis of control group.The levels of estradiol(E_(2)) on the 2nd and 7^(th)~8^(th) days of menstruation,1~2 days after follicles,and7~10 days after ovulation between the two groups,the occurrence of ovulation phenomena,luteinizing hormone(LH),and ovulation 7~10 days after progesterone(Prog) and pregnancy,color Doppler ultrasound monitors the thickness of the endometrium during ovulation and the cervical score during periovulation were compared between the two groups.Results There was no significant difference in serum E_(2) level between the two groups on the 2^(nd) day of menstruation(P>0.05);on the 7^(th)~8^(th) day of menstruation and 7~10 days after ovulation,the serum E_(2) level of observation group was significantly higher than control group,and it was 1~2 days after ovulation(P<0.05);there was no significant difference between the two groups on LH levels when ovulation occurred and Prog levels 7~10 d after ovulation(P>0.05);the thickness of the endometrium during ovulation and the cervical score during periovulation of observation group were higher than control group(P<0.05);both groups were observed for 3 cycles,there was no significant difference in preqrancy rate between the two groups(P>0.05).Conclusion Modified Huyang Decoction can significantly improve the level of sex hormones,endometrial thickness and cervical score in patients with small follicle ovulation,promote follicular development,and increase pregnancy rate;color Doppler ultrasound monitoring plays an important role in judging the curative effect of patients with small follicle ovulation.
