生化检验项目对肝硬化的诊断价值分析 被引量:3

摘要 目的分析生化检验项目对肝硬化的诊断价值。方法选取75例肝硬化患者作为肝硬化组,另选取同期75例接受体检的健康人作为对照组。两组均分别使用快速血糖仪和常规生化仪进行生化项目检验。比较肝硬化组与对照组各项生化检验项目检测结果,不同分级肝硬化患者各项生化检验项目检测结果。结果肝硬化组胆碱酯酶(CHE)(2411.9±750.2)U/L、白蛋白(ALB)(22.5±3.0)g/L均低于对照组的(7533.9±2234.8)U/L、(44.1±7.3)g/L,碱性磷酸酶(ALP)(185.3±29.1)U/L、γ-谷氨酰转肽酶(γ-GT)(180.7±25.4)U/L、丙氨酸氨基转移酶(ALT)(58.5±12.1)U/L、谷草转氨酶(AST)(93.7±14.2)U/L、总胆红素(TBIL)(88.2±15.4)μmol/L及直接胆红素(DBIL)(36.5±8.8)μmol/L高于对照组的(72.8±12.4)U/L、(18.8±7.2)U/L、(16.1±3.1)U/L、(16.3±3.1)U/L、(12.5±5.0)μmol/L、(3.2±0.8)μmol/L,差异有统计学意义(P<0.05)。C级和B级肝硬化患者CHE、ALB水平均低于A级,ALP、γ-GT、ALT、AST、TBIL及DBIL水平高于A级,差异有统计学意义(P<0.05);C级肝硬化患者CHE、ALB水平均低于B级,ALP、γ-GT、ALT、AST、TBIL及DBIL水平高于B级,差异有统计学意义(P<0.05)。结论生化检验项目对肝硬化的诊断价值较高,而且与肝硬化的病情严重程度密切相关,可以为疾病的诊断、分级及治疗提供可靠的指导依据。 Objective To analyze the value of biochemical test items in the diagnosis of cirrhosis.Methods 75 patients with cirrhosis during the same period were selected as the liver cirrhosis group,and 75 healthy subjects who received physical examination during the same period were selected as the control group.Both groups were tested for biochemical items using rapid glucose meter and conventional biochemical meter.Comparison was made on test results of various biochemical test items between the cirrhosis group and the control group,and the test results of various biochemical test items in patients with different grades of liver cirrhosis.Results The cholinesterase(CHE)(2411.9±750.2)U/L and albumin(ALB)(22.5±3.0)g/L of the cirrhosis group were lower than(7533.9±2234.8)U/L and(44.1±7.3)g/L of the control group;the alkaline phosphatase(ALP)(185.3±29.1)U/L,γ-glutamyl transpeptidase(γ-GT)(180.7±25.4)U/L,alanine aminotransferase(ALT)(58.5±12.1)U/L,aspartate aminotransferase(AST)(93.7±14.2)U/L,total bilirubin(TBIL)(88.2±15.4)μmol/L and direct bilirubin(DBIL)(36.5±8.8)μmol/L of the cirrhosis group were higher than(72.8±12.4)U/L,(18.8±7.2)U/L,(16.1±3.1)U/L,(16.3±3.1)U/L,(12.5±5.0)μmol/L,(3.2±0.8)μmol/L of the control group;all the differences were statistically significant(P<0.05).The levels of CHE and ALB patients with grade C and grade B cirrhosis were lower than those with grade A cirrhosis,and the levels of ALP,γ-GT,ALT,AST,TBIL and DBIL were significantly higher than those with grade A cirrhosis.All the differences were statistically significant(P<0.05).The levels of CHE and ALB in patients with grade C cirrhosis were lower than those with grade B cirrhosis,and the levels of ALP,γ-GT,ALT,AST,TBIL and DBIL were significantly higher than those with grade B cirrhosis.All the differences were statistically significant(P<0.05).Conclusion Biochemical test items have high value in the diagnosis of cirrhosis,and are closely related to the severity of liver cirrhosis,which can provide reliable guidance for the diagnosis,classification and treatment of the disease.
