老年风湿性心脏病伴心房颤动行瓣膜置换术后采用胺碘酮治疗的效果

Effect of amiodarone on elderly patients with rheumatic heart disease and atrial fibrillation after valve replacement

目的探讨老年风湿性心脏病伴心房颤动行瓣膜置换术后采用胺碘酮治疗的效果。方法130例老年风湿性心脏病伴心房颤动患者为研究对象,均行瓣膜置换术,随机分为丙组与丁组,每组65例。丙组术后采用常规治疗,丁组术后在丙组的基础上给予胺碘酮治疗。比较两组患者的心房颤动转复、窦性心率维持情况,重症加强护理病房(ICU)治疗时间、住院时间。结果治疗后,丁组患者的心房颤动转复率、窦性心率维持率分别为84.6%、92.3%,均高于丁组的69.2%、80.0%,差异具有统计学意义(P<0.05)。丁组患者的ICU治疗时间、住院时间分别为(1.9±1.4)、(7.1±2.1)d,短于丙组的(2.5±1.6)d、(13.2±1.8)d,差异具有统计学意义(P<0.05)。结论老年风湿性心脏病伴心房颤动行瓣膜置换术后采用胺碘酮治疗,可以提高心房颤动转复率、窦性心率维持率,有效缩短患者的ICU治疗时间、住院时间,值得临床推广与应用。

Objective To discuss the effect of amiodarone on elderly patients with rheumatic heart disease and atrial fibrillation after valve replacement.Methods A total of 130 elderly patients with rheumatic heart disease and atrial fibrillation after valve replacement were studied,and randomly divided into group C and group D,with 65 cases in each group.Group C was treated with conventional therapy after surgery,and group D was treated with amiodarone on the basis of group C after surgery.Both groups were compared in terms of conversion of atrial fibrillation,maintenance of sinus heart rate,duration of intensive care unit(ICU)treatment,and hospitalization time.Results After treatment,the conversion rate of atrial fibrillation and maintenance rate of sinus heart rate in group D were 84.6%and 92.3%,respectively,which were higher than 69.2%and 80.0%in group C,and the difference was statistically significant(P<0.05).The ICU treatment time and hospitalization time in group D were(1.9±1.4)and(7.1±2.1)d,respectively,which were shorter than(2.5±1.6)and(13.2±1.8)d of group C,and the differences were statistically significant(P<0.05).Conclusion For elderly patients with rheumatic heart disease and atrial fibrillation,the application of amiodarone after valve replacement can improve the conversion rate of atrial fibrillation and maintenance rate of sinus heart rate,and effectively shorten the ICU treatment time and hospitalization time of patients,which is worthy of clinical promotion and application.
