2020年我院住院患者抗菌药物使用情况分析

Analysis of the use of antibacterial drugs in inpatients in our hospital in 2020

目的对本院住院患者抗菌药物应用情况进行分析,旨在规范全院抗菌药物的使用,促进合理用药。方法采用东华标准版数字化医院信息管理系统随机抽取儿科、妇科、产科各400份病历,共1200份,分析抗菌药物金额占比、用药频度(DDDs)、围手术期预防使用抗菌药物情况、抗菌药物使用率等。结果2020年本院抗菌药物金额占全部药品消耗金额的23.05%,其中限制级占比15.37%,使用金额和DDDs排序第一的均为五水头孢唑林钠。Ⅰ类切口预防使用抗菌药物3.09%,预防使用合理率为97.40%。预防使用不合理主要包括预防时机不合理6例、抗菌药物品种选择不合理6例、疗程不合理4例。儿科、妇科、产科抗菌药物使用率分别为77.50%、58.00%、48.00%。结论本院抗菌药物使用基本合理,但仍存在抗菌药物使用率偏高、部分病例抗菌药物预防及治疗不合理等情况。

Objective To analyze the application of antibacterial drugs in inpatients in this hospital aims to standardize the use of antibacterial drugs in our hospital and promote the rational use of drugs.Methods 400 medical records of pediatrics,gynecology and obstetrics were randomly selected by digital hospital information management system of Donghua standard edition,a total of 1200 records.The proportion of antibacterial drug,the defined daily dose system(DDDs),the perioperative prophylactic use of antimicrobial agents,and the use rate of antibacterial drugs were analyzed.Results In 2020,the amount of antimicrobial drugs in our hospital accounted for 23.05%of the total amount of drug consumption,of which 15.37%was restricted,and the first ranking of both the amount used and DDDs was cefazolin sodium pentahydrate.The prophylactic use of antimicrobial agents for classⅠincision was 3.09%,and the reasonable rate of prophylactic use was 97.40%.The unreasonable use of prophylaxis mainly included 6 cases of unreasonable timing of prophylaxis,6 cases of irrational selection of antibacterial drugs,and 4 cases of irrational course of treatment.The use rates of antibacterial drugs in pediatrics,gynecology,and obstetrics were 77.50%,58.00%,and 48.00%,respectively.Conclusion The use of antimicrobial agents in our hospital is basically reasonable,but the utilization rate of antimicrobial agents is still high,and the prevention and treatment of antimicrobial agents in some cases are unreasonable.
