超声引导下麦默通微创旋切术治疗乳腺良性肿瘤效果的Meta分析

Meta analysis of ultrasound-guided mammotome minimally invasive surgery for benign breast masses

目的通过Meta分析探讨超声引导下麦默通微创旋切术与传统开放手术对乳腺良性肿瘤的治疗效果。方法采用计算机检索中国知网、万方、维普、PubMed等数据库,收集自建库起至2020年12月1日有关两种方法行乳腺良性肿瘤切除的文献。由两名研究人员对文献进行筛选、资料信息提取以及证据质量评价,应用RevMan 5.3软件进行Meta分析。结果30篇研究被纳入,共5189例参与者,其中微创手术组2594例,开放手术组2595例,Meta分析结果显示:微创手术组在手术时间[SMD=-3.71,95%CI(-4.33,-3.10),P<0.00001]、术中出血量[SMD=-5.04,95%CI(-5.74,-4.35),P<0.00001]、愈合时间[SMD=-3.21,95%CI(-3.63,-2.79),P<0.00001]、并发症例数[OR=0.21,95%CI(0.16,0.28),P=0.43]四个方面均优于开放手术组。结论超声引导下麦默通微创旋切术治疗乳腺良性肿瘤,在手术时间、术中出血量、愈合时间和并发症四个方面均优于传统开放手术。

To explore the effect of ultrasound-guided mammotome minimally invasive surgery and traditional open surgery on the treatment of benign breast tumors by meta-analysis.Methods The databases of CNKI,Wanfang,Weipu and PubMed were searched by computer to collect the literatures about the two methods of breast benign tumor resection from the establishment of the database to December 1,2020.Two researchers screened literatures,extracted data and evaluated the quality of evidence.Revman 5.3 software was used for meta-analysis.Results 30 studies were included,with 5189 participants,including 2594 cases in the minimally invasive surgery group and 2595 cases in the open surgery group.The results of meta-analysis showed that the minimally invasive surgery group was better than the open surgery group in terms of operation time[SMD=-3.71,95%CI(-4.33,-3.10),P<0.00001],intraoperative blood loss[SMD=-5.04,95%CI(-5.74,-4.35),P<0.00001],healing time[SMD=-3.21,95%CI(-3.63,-2.79),P<0.00001]and the number of complications[OR=0.21,95%CI(0.16,0.28),P=0.43].Conclusion The ultrasound-guided Mammotome minimally invasive surgical treatment of benign breast tumors is superior to traditional open surgery in terms of operation time,intraoperative blood loss,healing time and complications.
