实时剪切波弹性成像评估正常女性胸小肌硬度的信度研究

摘要 目的:探讨实时剪切波弹性成像(SWE)评价正常女性胸小肌硬度的可重复性和一致性。方法:收集60例健康女性志愿者,由医师A、B分别对其双侧胸小肌进行硬度检测,其中医师A重复检测1次,采集并存储所有图像,收集整理双侧胸小肌的平均杨氏模量值和平均剪切波速度。结果:组内相关系数(ICC)显示,2名医师检测双侧胸小肌平均杨氏模量值的一致性均为优秀,其中ICC_(右)=0.906,ICC_(左)=0.783(均P<0.05);2名医师检测右侧胸小肌平均剪切波速度一致性为优秀,ICC_(右)=0.844(P<0.05),检测左侧胸小肌平均剪切波速度一致性为良好,ICC_(左)=0.695(P<0.05)。医师A 2次检测双侧胸小肌平均杨氏模量值及平均剪切波速度的重复测量一致性均为优秀,杨氏模量值ICC_(右)=0.910,ICC_(左)=0.886;剪切波速度ICC_(右)=0.894,ICC_(左)=0.814(均P<0.05)。医师A检测右侧与左侧胸小肌的平均杨氏模量值及平均剪切波速度差异均无统计学意义(均P>0.05);医师B检测右侧与左侧胸小肌的平均杨氏模量值及平均剪切波速度差异均无统计学意义(均P>0.05)。结论:SWE评估正常女性双侧胸小肌硬度的信度较高,双侧胸小肌平均杨氏模量值和平均剪切波速度均无差异。 Objective:To investigate the repeatability and consistency of real-time shear wave elastography(SWE)in evaluating the hardness of pectoralis minor muscle in normal women.Methods:Sixty healthy female volunteers were involved in the study.Physician A and B were collected to test the hardness of pectoralis minor muscles of all women,and physician A repeated the measurement once.The images of all cases were collected and stored,and the average Young’s modulus value and average shear wave velocity(SWV)of pectoralis minor muscles were also collected and sorted.Results:The intraclass correlation coefficient(ICC)showed that the consistency of the average Young’s modulus values detected by the two doctors was excellent,with ICC_(right) of 0.906 and ICC_(left) of 0.783(P<0.05).The consistency of the average SWV of the right pectoralis minor muscle detected by the two doctors was excellent(ICC_(right)=0.844)(P<0.05),and the consistency of that of the left pectoralis minor muscle was good(ICC_(left)=0.695)(P<0.05).The repeated measurement consistency of the average Young’s modulus value and the average SWV in bilateral pectoralis minor muscles detected by physician A was excellent,the Young’s modulus value with ICC_(right) of 0.910 and ICC_(left) of 0.886,SWV with ICC_(right) of 0.894 and ICC_(left) of 0.814(all P<0.05).There were no significant differences in the average Young’s modulus value and average SWV between the right and left pectoralis muscles which were detected by physician A(P>0.05).While there were significant differences detected by physician B(P>0.05).Conclusions:SWE has a good reliability to evaluate the pectoralis muscles hardness of normal women,and the average Young’s modulus value and the average SWV in bilateral pectoralis minor muscles have no differences.
