摘要
目的:探讨行有创颅内压监测的高血压脑出血患者术后出现颅内感染的相关危险因素。方法:收集216例行颅内压探头置入术的高血压脑出血患者的相关临床资料,通过单因素及多因素Logistic回归分析年龄、性别、是否脑室积血、手术方式、手术时间、颅内压探头位置、颅内压探头留置时间、探头类型、肺部感染、术前血糖及国际标准化比值(international normalized ratio,INR)与术后颅内感染的关联。结果:术后共发生颅内感染42例(19.44%),颅内压探头留置时间[比值比(odds ratio,OR)=6.051,95%置信区间(confidence interval,CI):2.587~14.153]、术前血糖(OR=7.493,95%CI:3.281~17.116)及开颅手术(OR=2.429,95%CI:1.060~5.569)为患者术后颅内感染的独立危险因素。结论:颅内感染是高血压脑出血术后常见并发症,而术后颅内压探头留置时间、术前血糖及开颅手术是术后颅内感染的独立危险因素。
Objective:To explore the related factors of intracranial infection in patient with hypertensive intracerebral hemorrhage who received intracranial pressure monitoring.Methods:The relevant clinical data of 216 patients with hypertensive intracerebral hemorrhage treated by intracranial pressure probe implantation were collected.Univariate and multivariate Logistic regression were used to analyze the relationship between age,gender,whether intraventricular hematocele,operation mode,operation time,intracranial pressure probe position,intracranial pressure probe retention time,probe type,pulmonary infection,preoperative blood glucose and international normalized ratio(INR),and postoperative intracranial infection.Results:The intracranial infection rate was 19.44%in this study.Among the variable parameters,probe retention time[odds ratio(OR)=6.051,95%confidence interval(CI):2.587-14.153],preoperative glucose(OR=7.493,95%CI:3.281-17.116)and craniotomy(OR=2.429,95%CI:1.060-5.569)were independent risk factors for postoperative intracranial infection.Conclusion:Intracranial infection was a common post-operative complication of hypertensive intracerebral hemorrhage,and post-operative intracranial pressure probe retention time,preoperative glucose and craniotomy were independent risk factors of postoperative intracranial infection.
