PCI对老年冠心病患者心功能的影响

Effect of PCI on cardiac function in elderly patients with coronary heart disease

目的分析经皮冠状动脉介入治疗(PCI)对老年冠心病患者心功能的影响。方法134例老年冠心病患者为研究对象,按照治疗方法不同分为对照组(72例)及观察组(62例)。对照组患者接受常规药物治疗,观察组患者接受PCI治疗。对比两组患者的治疗效果,治疗前后的心功能指标[左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)],出院后1年的预后情况。结果观察组患者的总有效率为93.55%,高于对照组的72.22%,差异具有统计学意义(P<0.05)。治疗后,观察组患者的LVEF(55.44±5.18)%高于对照组的(47.86±5.66)%,LVESD(37.88±4.08)mm、LVEDD(45.66±5.16)mm小于对照组的(41.16±4.40)、(50.16±4.28)mm,差异具有统计学意义(P<0.05)。观察组患者的再入院率6.45%、疾病复发率16.13%、因病死亡率0均低于对照组的27.78%、33.33%、6.94%,差异具有统计学意义(P<0.05)。结论老年冠心病患者开展PCI治疗能取得满意的治疗效果以及远期预后,改善患者的心功能,值得在临床中进一步推广和应用。

Objective To analyze the effect of percutaneous coronary intervention(PCI)on cardiac function in elderly patients with coronary heart disease.Methods A total of 134 elderly patients with coronary heart disease as study subjects were divided into control group(72 cases)and observation group(62 cases)according to different treatment methods.Patients in the control group received conventional drug therapy,and patients in the observation group received PCI therapy.The therapeutic effect,cardiac function indexes[left ventricular ejection fraction(LVEF),left ventricular end systolic diameter(LVESD),left ventricular end diastolic diameter(LVEDD)]before and after treatment,and the prognosis at 1 year after discharge were compared between the two groups.Results The total effective rate of patients in the observation group was 93.55%,which was higher than 72.22%in the control group,and the difference was statistically significant(P<0.05).After treatment,the LVEF(55.44±5.18)%of the observation group was higher than(47.86±5.66)%of the control group,and the LVESD(37.88±4.08)mm and LVEDD(45.66±5.16)mm were less than(41.16±4.40)and(50.16±4.28)mm of the control group.All the differences were statistically significant(P<0.05).The re-admission rate 6.45%,disease recurrence rate 16.13%,and the mortality rate due to disease 0 of the observation group were lower than 27.78%,33.33%,and 6.94%of the control group,and the difference was statistically significant(P<0.05).Conclusion For elderly patients with coronary heart disease,PCI treatment can achieve satisfactory therapeutic effect and long-term prognosis,improve the cardiac function of patients,and is worthy of further clinical promotion and application.
