非酒精性脂肪性肝病合并2型糖尿病患者血清hs-CRP、TNF-α、IL-6、IL-10与鸢尾素水平及相关性分析

Analysis of serum hs-CRP,TNF-α,IL-6,IL-10 and irigenin levels in patients with non-alcoholic fatty liver disease complicated with type 2 diabetes mellitus

目的探究非酒精性脂肪性肝病(NAFLD)合并2型糖尿病(T2DM)患者血清中超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-10(IL-10)与鸢尾素水平及其相关性。方法选取我院2016年9月至2019年10月收治的NAFLD合并T2DM患者88例作为观察组,另选同期于我院体检的90名健康人群作为对照组,展开回顾性分析。观察两组血清hs-CRP、TNF-α、IL-6、IL-10与鸢尾素水平及其相关性。结果观察组的hs-CRP、TNF-α、IL-6水平显著高于对照组,IL-10、鸢尾素水平显著低于对照组,差异具有统计学意义(P<0.05)。Pearson相关性分析显示,鸢尾素水平与hs-CRP、TNF-α、IL-6水平呈负相关,与IL-10水平呈正相关(P<0.05);鸢尾素、IL-10水平与ALT、AST、空腹血糖水平、HbA1c呈负相关,而hs-CRP、TNF-α、IL-6水平与ALT、AST、空腹血糖水平、HbA1c呈正相关(P<0.05)。多因素Logistic回归分析结果显示,hs-CRP、TNF-α、IL-6为导致NAFLD合并T2DM的独立危险因素,IL-10与鸢尾素为保护因素(P<0.05)。结论NAFLD合并T2DM患者的血清hs-CRP、TNF-α、IL-6、IL-10与鸢尾素水平和正常人群存在显著差异,且上述指标与鸢尾素水平存在相关性。

Objective To explore the levels of serum high sensitivity C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-10(IL-10)and irigenin and their correlation in patients with non-alcoholic fatty liver disease(NAFLD)and type 2 diabetes mellitus(T2DM).Methods A total of 88 NAFLD patients with T2DM admitted in our hospital from September 2016 to October 2019 were selected as observation group,and 90 healthy people who received physical examination in our hospital during the same period were selected as control group for retrospective analysis.The levels of serum hs-CRP,TNF-α,IL-6,IL-10 and irigenin in the two groups and their correlation were observed.Results The levels of hs-CRP,TNF-αand IL-6 in the observation group were significantly higher than those in the control group,while the levels of IL-10 and irigenin were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that irigenin level were negatively correlated with hs-CRP,TNF-αand IL-6 levels,while was positively correlated with IL-10 level(P<0.05);irigenin and IL-10 levels were negatively correlated with ALT,AST,fasting blood glucose levels and HbA1c,while hs-CRP,TNF-αand IL-6 levels were positively correlated with ALT,AST,fasting blood glucose levels and HbA1c(P<0.05).The multivariate Logistic regression analysis showed that hs-CRP,TNF-α,IL-6 were independent risk factors for NAFLD and T2DM,IL-10 and irigenin were protective factors(P<0.05).Conclusion The levels of serum hs-CRP,TNF-α,IL-6,IL-10 and irigenin in patients with NAFLD and T2DM are significantly different from those in the normal population,and there is a correlation between the above indicators and irigenin.
