多模态血管超声成像在脑卒中高危人群中的应用价值

Application value of ultrasound multi-mode angiography in high-risk population of stroke

目的:探讨经颅彩色多普勒超声(TCCS)联合颈动脉超声多模态血管成像评估脑卒中高危人群的效果。方法:选取某院收治的脑卒中高危人群440例作为研究对象,均行经颅彩色多普勒超声和颈部血管超声检查,并收集其临床资料,统计分析头颈动脉超声检查结果及脑卒中高危因素。结果:年龄≥60岁脑卒中高危人群的内中膜增厚、斑块形成、颈动脉狭窄或闭塞及颅内椎基底动脉狭窄或闭塞发生率高于40~60岁患者(P<0.05);不同性别脑卒中高危人群内中膜增厚、斑块形成、颈动脉狭窄或闭塞及大脑中动脉狭窄或闭塞发生率比较差异有统计学意义(P<0.05);经单因素和多因素Logistic回归分析显示,合并有高血压、冠心病、明显超重或肥胖及年龄≥60岁是导致脑卒中高危人群颈动脉斑块形成的高危因素。结论:经颅彩色多普勒超声联合颈动脉超声多模态血管成像在脑卒中风险评估中具有重要的诊断价值,值得临床推广应用。高血压、冠心病、明显超重或肥胖、年龄≥60岁是脑卒中高危人群颈动脉斑块形成的高危因素,应引起重视。

Objective:To investigate the evaluation effect of transcranial color Doppler ultrasound combined with carotid artery ultrasound multi-mode angiography in high-risk population of stroke.Methods:A total of 440 patients at high risk of stroke admitted in a hospital were selected as the research objects.All of them underwent transcranial color Doppler ultrasound and cervical vascular ultrasound examination.Their clinical data were collected,and the results of head carotid artery ultrasound examination and high risk factors of stroke were statistically analyzed.Results:The incidence of carotid artery intima-media thickening,plaque formation,stenosis or occlusion and intracranial vertebrobasilar artery stenosis or occlusion in high-risk stroke populations aged over 60 years was significantly higher than that of patients aged 40~60 years(P<0.05);there were statistically significant differences in the incidence of carotid artery intima-media thickening,plaque formation,stenosis or occlusion,and middle cerebral artery stenosis or occlusion between male and female patients(P<0.05);logistic regression showed that hypertension,coronary heart disease,significantly overweight or obesity,and over 60 years of age were high-risk factors leading to high-risk status of stroke and carotid plaque formation.Conclusion:Transcranial color Doppler ultrasound combined with carotid ultrasound multi-mode angiography has important diagnostic value in stroke risk assessment and is worthy of clinical application.Hypertension,coronary heart disease,overweight or obesity,and over 60 years of age are high-risk factors for stroke and should be paid attention to.
