FRD联合Care HPV在大同地区宫颈癌筛查中的临床研究

The Clinical Research of FRD Combined with Care HPV on Cervical Screening in Datong City

目的探究叶酸受体介导的宫颈特殊染色(FRD)联合宫颈癌快速筛查(Care HPV)在大同地区宫颈癌筛查中的临床应用。方法以自身为对照采用FRD联合Care HPV与液基细胞检查(TCT)联合Aptima HPV同时对2000例21~65岁有性生活的女性进行宫颈癌筛查,对比两种方法的检出率、病理符合率、漏诊率、敏感度、特异度、阳性预测值和阴性预测值。结果检出率、漏诊率、病理符合率、敏感度和阳性预测值在FRD、TCT、Care HPV和Aptima HPV组间差异无统计学意义,在FRD+Care HPV和TCT+Aptima HPV组间差异无统计学意义,而在FRD+Care HPV(或TCT+Aptima HPV)与FRD(或TCT、或Care HPV、或Aptima HPV)差异有统计学意义。特异度和阴性预测值在FRD、TCT、Care HPV、Aptima HPV、FRD+Care HPV和TCT+Aptima HPV组间差异无统计学意义。结论FRD联合Care HPV与TCT联合Aptima HPV对宫颈癌筛查检出率、病理符合率、漏诊率、敏感度和阳性预测值相当,但检测费用和检测时间显著降低,适合在经济条件和医疗设施相对落后地区推广。

Objective To investigate the clinical application effect of folate receptor-mediated tumor detection(FRD)combined with Care HPV on cervical cancer screening in Datong city.Methods Two thousand sexually active women aged from 21 to 65 years old were screened for cervical cancer via FRD combined with Care HPV and Thin-Cytologic Test(TCT)combined with Aptima HPV.The detectable rate,coincidence rate in pathology,sensitivity,specificity,positive predictive value,negative predictive value of the two methods were compared.Results There were no significant differences in detection rate,coincidence rate in pathology,misdiagnosis rate,sensitivity and positive predictive value among FRD,TCT,CARE HPV and Aptima HPV groups,and there was no differences in sensitivity and positive predictive value between FRD+CARE HPV and TCT+Aptima HPV groups,while there was differences between FRD+CARE HPV(or TCT+Aptima HPV)and FRD(or TCT,or CARE HPV,or Aptima HPV)groups.There were no differences in specificity and negative predictive value among FRD,TCT,CARE HPV,APTIMA HPV,FRD+CARE HPV and TCT+APTIMA HPV groups.Conclusion FRD combined with Care HPV and TCT combined with Aptima HPV possess the same detection rate,coincidence rate in pathology,misdiagnosis rate,sensitivity and positive predictive value for cervical cancer screening,but the cost and detection time of FRD combined with CARE HPV are remarkably reduced,be especially suited to implement in the areas with relatively backward economic conditions and medical facilities.
