小儿肾积水术后双J管留置时间临床观察

Clinical observation of double J tube indwelling time of children with hydronephrosis after operation

目的观察小儿腹腔镜下肾盂成形术后不同双J管留置时间对其留置并发症的影响。方法收集2018年6月~2020年10月于潍坊市人民医院行腹腔镜肾盂成形术患儿56例,其中常规保留双J管时间4~8周为A组,延期至12~16周为B组,对比两组患儿并发症发生情况,分析双J管留置时间与并发症发生关系。结果两组均有不同程度并发症发生,其中A组与B组相比,各种并发症发生率分别为,腰痛:26.32%vs 55.56%;肉眼血尿:18.42%vs 16.67%;膀胱刺激征:5.56%vs 27.78%;泌尿系感染:7.89%vs 33.33%;导管移位:7.9%vs 11.11%;取管困难:5.26%vs 16.67%;两组均未见吻合口尿瘘发生。其中A组患儿术后腰痛、膀胱刺激征、泌尿系感染等发生率明显低于B组,差异有统计学意义(P<0.05)。结论双J管留置时间的有效管理可减少相应并发症发生率。

Objective To study and compare the effect of different indwelling time of double-J tube on indwelling complications after children laparoscopic pyeloplasty.Methods Fifty-six cases with children laparoscopic pyeloplasty in Weifang People’s Hospital from June 2018 to October 2020 were collected.The routine retention time of double J tube was 4~8w in group A,and the time was extended to 12~16w for group B.The incidence of complications between the two groups was compared,and the relationship between the retention time of double J tube and complications was analyzed.Results There were different degrees of complications in the two groups.Compared with group B,the incidence of complications in group A was:lumbago 26.32%vs 55.56%;gross hematuria:18.42%vs 16.67%;bladder irritation:5.56%vs 27.78%;urinary tract infection:7.89%vs 33.33%;catheter displacement:7.9%vs 11.11%;difficulty in taking out the catheter:5.26%vs 16.67%;there was no anastomotic fistula in the two groups.The incidence of postoperative lumbago,bladder irritation and urinary tract infection in group A was significantly lower than that in group B(P<0.05).Conclusion The effective management of double J tube indwelling time can reduce the incidence of complications.
