摘要
目的研究自由体位分娩联合无保护会阴接生技术对分娩结局的影响。方法选取2019年1—5月于产科足月分娩的160例产妇为研究对象,按照随机数字表法将所有产妇分为对照组和观察组,各80例。对照组采用传统助产技术协助分娩,观察组采用自由体位管理联合无保护会阴接生技术进行干预。比较2组的剖宫产率、产程时间、会阴侧切率、会阴裂伤情况、产后出血率及新生儿并发症发生率。结果观察组剖宫产率低于对照组(χ^(2)=4.379,P=0.036);总产程时间明显短于对照组(t=9.118,P<0.001);产妇会阴侧切率低于对照组(χ^(2)=10.025,P=0.002);2组产妇会阴裂伤率、产后出血率及新生儿并发症发生率比较,差异均无统计学意义(P>0.05)。结论自由体位分娩联合无保护会阴接生技术可降低剖宫产率,缩短产程时间,降低会阴侧切率,不增加会阴裂伤率、产后出血率及新生儿并发症发生率,利于母婴健康,值得临床推广。
Objective To explore the outcomes of free position delivery combined with unprotected perineum delivery.Methods Totally 160 pregnant women who delivered in our hospital in the first five months of 2019 were selected and randomly divided into a control group and an observation group,each of 80.The control group delivered using the traditional midwifery technique,while the observation group used the free position delivery combined with unprotected pericardium delivery.Then,the cesarean section rate,labor time,perineum side cut rate,perineum laceration rate,perineum integrity rate,postpartum hemorrhage rate and neonatal complication rate were compared between the control group and the observation group.Results The cesarean section rate and perineum side cut rate of the observation group was significantly lower than that of the control group(χ^(2)=4.379,P=0.036;χ^(2)=10.025,P=0.002),and the total labor time of the former was significantly shorter than the latter(t=9.118,P<0.001).However,no significant differences were found in the other measurements(P>0.05).Conclusion Free position delivery combined with unprotected perineum delivery can reduce the cesarean section rate,shorten the time of labor,and lower the rate of lateral perineum resection.It is beneficial to the maternal and neonatal health,and is worthy of clinical popularizing.
