老年糖尿病患者居家安全评价指标体系的构建

Construction of home safety evaluation index system for elderly diabetic patients

目的探索建立适用于老年糖尿病患者居家安全的评价指标体系。方法通过查阅文献及访谈法,拟定老年糖尿病居家患者安全指标基本框架,确定一级备选指标、二级备选指标。遴选评价专家,运用德尔菲法,对备选指标评价进行两轮专家咨询,评价专家意见的集中程度及专家的权威程度,确定糖尿病患者居家安全评价指标体系。结果拟定的患者安全指标基本框架包括9个一级备选指标和52个二级备选指标。15名专家对备选指标进行了两轮咨询,两轮专家积极系数均为100%,权威系数分别为0.71和0.79,最终确立的老年糖尿病患者居家安全评价指标体系包括10项一级指标、55项二级指标。10项一级指标分别是跌倒、压疮、用药错误、低血糖、糖尿病足、感染、烫伤、酮症酸中毒、心理异常及社会家庭支持,指标权重分别是10.14%、9.57%、10.00%、10.43%、10.14%、10.14%、10.00%、10.29%、9.43%及9.86%。结论初步构建了老年糖尿病患者居家安全评价指标体系,可为老年糖尿病患者居家照护提供具体可量化的安全评价标准。

Objective To explore the establishment of an evaluation index system for home safety in elderly diabetic patients.Methods Through literature review and interview methods,we drawn up the basic framework of safety index for elderly diabetic patients at home,and determine the primary alternative indicators and secondary alternative indicators.Select evaluation experts and conduct two rounds of expert consultation on the evaluation of alternative indicators by using Delphi method,evaluate the concentration degree and the authority of experts,and determine the evaluation index system of home safety for diabetic patients.Results The proposed basic framework of patient safety index includes 9 primary alternative indicators and 52 secondary alternative indicators.A total of 15 experts participated in two rounds of consultation.The positive coefficients of the two rounds of experts were 100%,and the authority coefficients were 0.71 and 0.79.The final home safety evaluation index system for elderly diabetic patients includes 10 primary indicators and 55 secondary indicators.The 10 primary indicators were falls,pressure ulcers,medication errors,hypoglycemia,diabetic foot,infection,scald,ketoacidosis,psychological abnormality and social family support.The weights were 10.14%,9.57%,10%,10.43%,10.14%,10.14%,10%,10.29%,9.43%and 9.86%respectively.Conclusion An evaluation index system of home safety for elderly diabetic patients has been preliminarily,which could provide a specific quantitative safety evaluation standards for home care of elderly diabetic patients.
