家庭功能和社会支持对胆道术后老年患者急性应激反应的影响

Effects of family function and social support on acute stress response in elderly patients after biliary tract surgery

目的探讨家庭功能与社会支持对胆道术后老年患者急性应激反应的影响。方法2018年10月~2019年10月,采取便利抽样法选取唐山市某三级甲等医院60岁以上胆道术后患者282例为研究对象,应用疼痛视觉模拟量表(VAS)、家庭功能评定量表(APGAR)、领悟社会支持评定量表(PSSS)、斯坦福急性应激反应量表(SASRQ)及自编的一般情况问卷进行调查。结果家庭功能良好、社会支持满意度高的胆道术后老年患者急性应激反应得分较低(P<0.05)。家庭功能和社会支持与胆道术后老年患者急性应激反应呈负相关(r=-0.615,P<0.05;r=-0.626,P<0.05)。多元逐步回归分析结果显示,文化程度、住院费用有无困难、性格、月收入、术后疼痛程度、家庭功能、社会支持是急性应激反应的影响因素,可解释急性应激反应86.5%的变异量。结论胆道术后老年患者急性应激反应发生率较高,其中家庭功能和社会支持影响胆道术后老年患者急性应激反应的发生。

Objective To investigate the effect of family function and social support on acute stress response of elderly patients after biliary tract operationthe.Methods From October 2018 to October 2019,282 patients over 60 years old after biliary tract surgery in a tertiary hospital in Tangshan were selected by convenience sampling method.The visual analog scale(VAS),family function assessment scale(APGAR),understanding social support rating scale(PSSS),Stanford acute stress reaction questionnaire(SASRQ)and the questionnaire of the general situation were used for investigation.Results The scores of acute stress state in elderly patients with good family function and satisfied social support group were lower(P<0.05).Family function and social support were negatively correlated with acute stress in elderly patients after biliary tract surgery(r=-0.615,P<0.05;r=-0.626,P<0.05).Multiple stepwise regression analysis,cultural degree,hospitalization expenses without difficulty,character,monthly income,postoperative pain degree,family functioning and social support were the influencing factors of acute stress response,which could explain 86.5%variation of acute stress response.Conclusion The incidence of acute stress reaction in elderly patients after biliary tract surgery is higher,family function and social support influence acute stress response in elderly patients after biliary tract surgery.
