急性咽喉肿痛刺血治疗选穴规律分析

Analysis of the regularity of acupoint selection in the treatment of acute throat-swelling diseases by pricking blood therapy

目的探讨刺血治疗急性咽喉肿痛的临床选穴特点。方法以“急性咽喉肿痛”、“刺血”为主要检索词检索中国期刊全文数据库(CNKI)、万方数据库(WF)和维普数据库(CQVIP)收录的相关临床文献,建立腧穴处方数据库,应用SPSS 20.0和Clementine 12.0数据挖掘软件进行穴位频次分析和关联规则分析。结果共检索到相关文献351篇,其中CNKI 175篇,WF 79篇,CQVIP 97篇;去重159篇,人工筛选去除57篇,最终纳入135篇。刺血治疗急性咽喉肿痛排名前五位的穴位是:少商(91次)、商阳(49次)、耳尖(43次)、咽喉患部(33次)、大椎(21次);穴位归经排名前三位的是:手太阴经、手阳明经、督脉;所选腧穴主要集中在掌指和跖趾关节以下的部位,特定穴中(井穴)应用频次最多(150次);穴位组合中少商-商阳合用几率最大,其次为商阳-大椎-少商,再次为商阳-咽喉患部-少商。结论急性咽喉肿痛刺血以肺经、大肠经、督脉为主,多取井穴、耳穴、奇穴等咽喉反应点刺血,重症、顽症多局部刺血。

Objective To analyze the characteristics of clinical acupoint selection in the treatment of acute throat-swelling diseases by using pricking blood therapy based on data mining.Methods To search the clinical literature on the pricking blood therapy of acute throat-swelling diseases by computer,published in CNKI,WanFang and CQVIP database,the database of acupoint prescription was established,the frequency and association rule of acupoints were analyzed by SPSS 20.0 and Clementine12.0 data mining software.Results A total of 351 relevant literatures were retrieved,including 175 literatures in CNKI,79 literatures in WF and 97 literatures in VIP.159 articles were eliminated,57 articles were manually screened,and 135 articles were finally included.The top five acupoints in the treatment of acute laryngopharyngeal pain by acupuncture were Shaoshang(LU11,91 times),Shangyang(LI1,49 times),Erjian(EX-HN6,43 times),Ashi Points of the laryngopharyngeal disease(33 times)and Dazhui(DU14,21 times),the top three meridians were the Lung meridians(LU)and the Large Intestine meridians(LI)and the Governor Vessel(GV),the selected acupoints mainly focused on the position below the metacarpophalangeal and metatarsophalangeal joints,Well Points(150 times)were the most frequents in special acupoints,the combination of Shaoshang(LU11)-Shangyang(LI1)was the most likely to be used,followed by the combination of LI-DU14-LI1,and then the combination of LI1-Ashi-LU11 points of the laryngopharyngeal disease.Conclusion Acupoint selection in the treatment of acute throat-swelling diseases by using pricking blood therapy are mainly Lung meridians,Large Intestine meridians and Governor Vessel medirians,and the characteristics of acupoint selection are that Well Points,Auricular Points and Extra Acupoint,and the Ashi Points are usually used in the serious laryngopharyngeal disease.
