摘要
目的探讨壳聚糖凝胶对下肢深静脉血栓形成后溃疡的有效性及安全性。方法收集2019年1月至2020年12月北京中医药大学东直门医院收治的72例下肢深静脉血栓形成后溃疡患者的临床资料,根据治疗方法不同将患者分为观察组和对照组,每组36例。两组患者均采取标准压力治疗,观察组患者外用壳聚糖凝胶后外敷凡士林纱布敷料治疗,对照组患者仅外敷凡士林纱布敷料治疗。治疗前、治疗1周后、治疗3周后,比较两组患者的溃疡面积、溃疡愈合情况、创面pH值。结果治疗1周后、治疗3周后,观察组患者的溃疡面积均小于对照组患者,差异均有统计学意义(P<0.05)。治疗3周后,观察组患者的溃疡愈合率为55.56%(20/36),高于对照组患者的30.56%(11/36),差异有统计学意义(P<0.05)。治疗3周后,观察组患者的创面pH值低于对照组患者,差异有统计学意义(P<0.05)。两组患者的安全性指标均无明显异常变化。结论壳聚糖凝胶能够提高下肢深静脉血栓形成后溃疡的愈合率,降低创面pH值,且安全性较高。
Objective To investigate the efficacy and safety of chitosan gel in ulcer secondary to post-deep venous thrombosis of lower limb.Method Clinical data of 72 patients with post-thrombotic ulcer in deep veins of lower limbs admitted in Dongzhimen Hospital of Beijing University of Chinese Medicine from from January 2019 to December 2020 was collected.According to different treatment methods,all patients were divided into observation group(n=36,treated with chitosan gel+topical vaseline gauze)and control group(n=36,treated with topical vaseline gauze).The ulcer area,ulcer healing rate and wound pH value of the patients in both groups were compared before the treatment,1 week and 3 weeks after the treatment.Result The ulcer are after 1 week and 3 weeks of treatment in the experimental group was lower than that in the control group(P<0.05);after 3 weeks of treatment,the ulcer healing rate in the experimental group was higher than that in the control group[55.56%(20/36)vs 30.56%(11/36),P<0.05];the wound pH value was lower than that in the control group(P<0.05).There was no significantly abnormal change for the safety of patients in both groups.Conclusion Chitosan gel can improve the ulcer healing rate of post-thrombotic ulcer in deep veins of lower limbs,reduce the wound pH value with higher safety.
