血清25-羟维生素D_(3)水平与2型糖尿病合并大血管病变的相关性研究 被引量:2

摘要 目的探讨血清25-羟维生素D_(3)[25(OH)D_(3)]水平与2型糖尿病合并大血管病变(MVC)的相关性。方法收集2018年6月至2020年6月沧州市中心医院收治的120例2型糖尿病患者的临床资料,53例合并MVC的2型糖尿病患者纳入MVC组,67例未合并MVC的2型糖尿病患者纳入对照组。2型糖尿病合并MVC的影响因素分析采用Logistic回归分析;对MVC组患者的年龄、病程、低密度脂蛋白胆固醇(LDL-C)、超敏C反应蛋白(hs-CRP)和MVC评分与血清25(OH)D_(3)水平进行相关性分析。结果MVC组患者年龄、LDL-C、hs-CRP高于对照组患者,病程长于对照组患者,血清25(OH)D_(3)低于对照组患者,差异有统计学意义(P<0.05)。MVC组患者血清25(OH)D_(3)水平与年龄、血清hs-CRP水平、MVC评分间呈高度负相关性,与病程和LDL-C水平间呈低度负相关性(P<0.01)。多因素回归分析结果显示,血清25(OH)D_(3)水平升高是2型糖尿病患者合并MVC的保护因素;年龄增长、病程延长、LDL-C和hs-CRP水平升高是2型糖尿病患者合并MVC的独立危险因素(P<0.05)。结论血清25(OH)D_(3)水平升高是2型糖尿病合并MVC的保护因素且25(OH)D_(3)水平与MVC评分具有负相关性。 Objective To study the relationship between serum 25-hydroxyvitamin D_(3)[25(OH)D_(3)]levels and macrovascular complications(MVC)in type 2 diabetes mellitus(T2 DM).Method The data of 120 T2 DM patients admitted to Cangzhou Central Hospital from June 2018 to June 2020 were retrospectively analyzed.A total of 53 T2 DM patients with combined MVC were included in the MVC group,and 67 T2 DM patients without MVC were included in the control group.The general data and clinical data of the two groups were compared.Logistic analysis was used to analyze the relationship between serum 25(OH)D_(3)and T2 DM combined with MVC.The correlation between age,course of disease,low density lipoprotein-cholesterol(LDL-C),hypersensitive C-reactive protein(hs-CRP),MVC score and serum 25(OH)D_(3)level was analyzed.Result The age,LDL-C,hs-CRP in MVC group were higher than those in the control group,the course of disease was longer than that in the control group,and the serum 25(OH)D_(3)was lower than that in the control group(P<0.05).In MVC group,serum 25(OH)D_(3)level was highly negatively correlated with age,serum hs-CRP level and MVC score,and low negatively correlated with course of disease and LDL-C level(P<0.01).Multivariate regression analysis showed that elevated serum 25(OH)D_(3)level was a protective factor for T2 DM with MVC.Age,duration,and elevated LDL-C and hs-CRP levels were independent risk factors for MVC in T2 DM patients(P<0.05).Conclusion The rise of serum 25(OH)D_(3)level is a protective factor for T2 DM combined with MVC and 25(OH)D_(3)level and MVC score have a negative correlation.
