EB病毒血清免疫球蛋白A检测在鼻咽癌筛查中的应用

Application of serological detection of EB virus immunoglobin A in nasopharyngeal carcinoma screening

目的评估EB病毒抗体Z反式激活因子(Zta)/免疫球蛋白A(IgA)、EB病毒核抗原1(EBNA1)/IgA和病毒衣壳抗原(VCA)/IgA抗体结果模式分析对鼻咽癌(NPC)风险评估的临床价值。方法共入组NPC患者662例、健康对照731例,临床筛查患者157例,采用酶联免疫吸附测定(ELISA)检测血清中EB病毒3种IgA抗体水平,分析不同结果模式在3组人群中的表现。结果NPC患者组出现单独1种抗体阳性结果模式的比例显著低于健康对照组,2种或3种抗体阳性的结果比例显著高于健康对照组,3种抗体全阳的结果模式在NPC组中出现的概率最高,占74.77%,阳性预测值为98.82%。结论3种抗体联合检测的结果模式有助于临床更好地评估疾病风险,并对NPC作出早期诊断。

Objective To evaluate the clinical value of EB virus antibody Z trans activator(Zta)/immunoglobulin A(IgA),EB virus nuclear antigen 1(EBNA1)/IgA and viral capsid antigen(VCA)/IgA in the risk assessment of nasopharyngeal carcinoma(NPC).Methods A total of 662 patients with NPC,731 healthy adults and 157 clinical screening patients were included.The levels of Zta/IgA,EBNA1/IgA and VCA/IgA to EB virus in serum were detected by enzyme-linked immunosorbent assay(ELISA),and the performance of different result patterns in the three groups was analyzed.Results The proportion of single antibody positive result mode in NPC group was significantly lower than that in healthy control group,and the proportion of two or three antibody positive results was significantly higher than that in normal control group.The probability of all three antibody positive result modes in NPC group was the highest,accounting for 74.77%,and the positive prediction rate was 98.82%.Conclusions The result mode of combined detection of Zta/IgA,EBNA1/IgA and VCA/IgA have referential value in diagnosing NPC.
