中医疗法治疗强直性脊柱炎病理性新骨形成的研究进展

Research Progress of Pathological New Bone Formation in TCM Treatment of Ankylosing Spondylitis

病理性新骨形成是强直性脊柱炎(ankylosing sporidylitis,AS)的重要特征之一。目前,中医疗法主要是通过控制腱端炎、抑制Wnt/β-catenin传导通路、调节骨形态发生蛋白信号通路及微小RNA(microRNA,miRNA)等途径干预AS病理性新骨形成。中医特色疗法众多,现有研究主要涉及中药汤剂、督脉灸、揿针、中医手法、中药活性成分等。中医疗法在治疗AS病理性新骨形成方面进行了广泛的研究,然而其相关机制并未完全明确,腱端炎与新骨形成之间的内在联系有待挖掘;穴位埋线、刮痧、刺络放血、拔罐等特色疗法干预AS病理性新骨形成的相关研究缺失;放射学变化是评价AS病理性新骨形成的可靠指标,但现有研究并未引入放射学评价指标。故今后需进一步加强对多种中医特色疗法及AS病理性新骨形成的相关分子机制的研究,同时在疗效评价中引入放射学评价指标,为中医疗法治疗AS病理性新骨形成提供更有力的证据。

Pathological new bone formation is one of the important features of ankylosing spondylitis(AS).At present,traditional Chinese medicine mainly interferes with pathological new bone formation in AS by controlling tendonitis,inhibiting the Wnt/β-catenin conduction pathway,regulating BMP signaling pathway and microRNA(miRNA).There are many characteristic therapies in traditional Chinese medicine,and the existing research mainly involves traditional Chinese medicine decoctions,Governor Vessel moxibustion,pressing needles,traditional Chinese medicine Tuina techniques,and active ingredients of traditional Chinese medicines.Traditional Chinese medicine has conducted extensive research on the treatment of pathological new bone formation in AS,but its related mechanism is not completely clear.The internal connection between tendinitis and new bone formation needs to be explored.Acupoint thread embedding,scraping,bloodletting,cupping,and other characteristic therapies are lacking in related research on the intervention of pathological new bone formation in AS;radiological changes are a reliable indicator for evaluating pathological new bone formation in AS,however,existing research does not introduce radiology Evaluation indexes.Therefore,in the future,it is necessary to further strengthen the research on various traditional Chinese medicine therapies and related molecular mechanisms of AS pathological formation of new bone.At the same time,radiological evaluation indicators should be introduced into the efficacy evaluation to provide stronger evidence for the treatment of pathological new bone formation in AS with traditional Chinese medicine.
