郑玉玲辨治大肠癌用药规律探析

Medication Pattern Analysis of Professor Zheng Yuling′s Treatment of Colorectal Cancer

目的:基于郑玉玲教授治疗大肠癌的门诊病例资料,采用数据挖掘方法,探析总结其用药组方规律。方法:通过肿瘤临床诊疗与患者管理一体化平台软件导出郑玉玲教授门诊的患者资料,对其病历信息、中药处方进行频数统计、聚类分析、因子分析、关联规则分析及复杂网络分析等,挖掘其中的用药规律和组方特点。结果:符合纳入条件的患者55例,纳入处方171张,高频方剂为人参健脾丸、薯蓣丸、枳实消痞汤、参苓白术散、芍药汤。将高频核心药物进行数据分析后,得到2类功效不同的药物组合、7组常用药对、3个模块群。核心药物以味甘、性温为主,辛、苦为次;归脾经、胃经居多,肺经、肝经次之;补虚药为主,消食药及利水渗湿药次之,各占41.62%、16.29%、12.04%。结论:郑玉玲教授认为大肠癌与脾胃虚弱或失调密切相关,治疗应以补脾和胃为主,佐以清热祛湿、调理气机、活血化瘀。

Objective:To analyze and summarize the pattern of prescriptions based on the data of Professor Zheng Yuling′s outpatient patients in the treatment of colorectal cancer with the data mining method.Methods:Through the integrated platform software of tumor clinical diagnosis,treatment,and patient management,the medical records of patients treated in the outpatient clinic of Professor Zheng Yuling were exported.Frequency statistics,cluster analysis,factor analysis,association rules analysis and complex network analysis,and other methods were used to conduct statistics on medical record information and TCM prescriptions,to explore the rules of medication and the characteristics of prescription.Results:55 eligible patients and 171 prescriptions were included.The high-frequency prescriptions were Renshen Jianpi Pill,Shuyu Pill,Zhishi Xiaopi Decoction,Shenling Baizhu Powder,Shaoyao Decoction.After applying data analysis on high-frequency core drugs,2 types of drug combinations with different efficacy,7 groups of common drug pairs,and 3 module groups were obtained.The nature and taste of core drugs were mainly warm,smooth,pungent,and bitter.Most of the meridians were channeled into the spleen and stomach,followed by the lung and liver channels.Among them,drugs for tonifying deficiency accounted for 41.62%,drugs for disinfecting food accounted for 16.29%,drugs for eliminating dampness and diuresis accounted for 12.04%.Conclusion:Professor Zheng Yuling believes that colorectal cancer is closely related to Spleen and Stomach dysfunction,and the treatment should mainly be invigorating the Spleen and Stomach,clearing heat and dampness,regulating Qi movement,and promoting blood circulation,and removing blood stasis.
