上海市浦东新区1176例产妇中医体质分布及与围产期抑郁的相关性分析

Correlation analysis of TCM constitution distribution and PND among 1176 parturients in Shanghai Pudong New Area

目的研究上海市浦东新区产妇中医体质分布、围产期抑郁发生率及与中医体质的相关性,为孕产期心理保健开展针对性的中医指导提供依据。方法对产妇进行问卷调查,采用《中医9种基本体质分类量表》和爱丁堡抑郁筛查量表(EPDS)进行体质分型和抑郁筛查,研究中医体质分布及与围产期抑郁的相关性。结果共纳入产妇1176例,体质占比前3位的分别为平和质(54.34%),阴虚质(14.46%),阳虚质(9.69%)。筛查发现抑郁者248例(21.09%),气郁质和气虚质产妇中抑郁者占比较高,分别占比75.61%、45.16%,平和质产妇中抑郁者占比最低(10.95%)。Logistic多元回归分析显示气郁质和气虚质为抑郁的危险体质,平和质为抑郁的保护体质。结论浦东新区产妇围产期抑郁的发生率为21.09%;气郁质和气虚质为围产期抑郁的危险体质,平和质为围产期抑郁的保护体质。

Objective To provide evidence for the targeted guidance of traditional Chinese medicine(TCM)during maternal health care by exploring the correlation between TCM constitution and the incidence of perinatal depression(PND)in Pudong New Area.Methods The parturients were surveyed by questionnaire,and the constitution typing and depression screening were performed by the 9 basic constitution classification scale of TCM and the edinburgh postnatal depression screening scale(EPDS)to study the distribution of TCM constitution and its correlation with PND.Results A total of 1176 parturients were included in this research.The top three physical conditions were Pinghe constitution(54.34%),Yinxu constitution(14.46%)and Yangxu constitution(9.69%).Screening showed 248 cases with depression(21.09%).The results showed that there were a large proportion of parturients with PND in Qiyu constitution and Qixu constitution,accounting for 75.61%and 45.16%,respectively.The proportion of depressed women in Pinghe constitution was the lowest(10.95%).Logistic multiple regression analysis showed that Qiyu constitution and Qixu constitution were the risk constitution for PND,Pinghe constitution was the protection constitution for PND.Conclusion The incidence of PND is 21.09%in Pudong New Area.Qiyu constitution and Qixu constitution were the risk constitutions,while Pinghe constitution was the protection constitution of PND.
