摘要
目的:观察纳米碳负显影技术在甲状腺癌根治术中对甲状旁腺的保护作用。方法:回顾性分析2018年1月至2020年1月该院收治的90例行甲状腺癌根治术患者的临床资料,按照术中是否应用纳米碳负显影技术分为对照组和观察组各45例。两组均采用甲状腺癌根治术治疗,对照组术中根据经验避免甲状旁腺损伤,观察组术中通过纳米碳负显影技术保护甲状旁腺。比较两组手术前后血钙水平、甲状旁腺激素(PTH)水平,甲状旁腺损伤/误切发生率、喉返神经损伤发生率以及复发率、转移率。结果:术后,观察组血钙和PTH水平均高于对照组,差异有统计学意义(P<0.05);观察组甲状旁腺损伤/误切、喉返神经损伤发生率均为2.2%,分别低于对照组的17.8%、20.0%,差异有统计学意义(P<0.05);两组复发率和转移率比较,差异均无统计学意义(P>0.05)。结论:纳米碳负显影技术应用于甲状腺癌根治术患者可保护其甲状旁腺,降低喉返神经损伤发生率,且对血钙和PTH水平的影响较小。
Objective:To observe protective effects of nano-carbon negative imaging technology on parathyroid glands during thyroid cancer surgeries.Methods:A retrospective analysis of the clinical data of 90 patients undergoing thyroid cancer surgeries admitted to the hospital from January 2018 to January 2020 was performed.According to whether the nano-carbon negative imaging technology was used during the surgery,they were divided into control group and observation group,45 cases in each group.Both groups were treated with radical thyroidectomy combined with lymph node dissection.In the control group,the parathyroid gland damage was avoided based on experience,while in the observation group,the parathyroid glands were protected by the nano-carbon negative imaging technology during the surgery.The blood calcium levels,the parathyroid hormone(PTH)levels,the incidence of parathyroid injury/miscut,the incidence of recurrent laryngeal nerve injury,the recurrence rate,and the metastasis rate were compared between the two groups before and after the surgery.Results:After the surgery,the blood calcium level and the PTH level of the observation group were higher than those of the control group,and the difference was statistically significant(P<0.05).The incidence of parathyroid injury/miscut in the observation group was 2.2%(1/45),which was lower than 17.8%(8/45)in the control group;the incidence of recurrent laryngeal nerve injury in the observation group was 2.2%(1/45),which was lower than 20.0%(9/45)of the control group;and the differences were statistically significant(P<0.05).However,there were no significant differences in the recurrence rate and the metastasis rate between the two groups(P>0.05).Conclusions:The nano-carbon negative imaging technology applied to the patients undergoing thyroid cancer surgeries can protect their parathyroid glands,reduce the incidence of recurrent laryngeal nerve injury,and have a small effect on the blood calcium and the PTH levels.
