摘要
目的分析支气管哮喘急性发作并肺部感染患者血清炎性因子、免疫球蛋白水平与预后的相关性。方法选取本院2018年1月-2020年1月收治的80例支气管哮喘急性发作并肺部感染患者(观察组)和80名健康志愿者(对照组)作为研究对象。观察组患者实施规范化治疗,比较两组血清白细胞介素-33(IL-33)、免疫球蛋白E(IgE)水平及肺功能,并分析IL-33、IgE与患者肺功能及预后的关系。结果观察组血清IL-33、IgE水平高于对照组,差异有统计学意义(P<0.05);观察组的IL-33、IgE、FEV1、FEV1%pred及FEV1/FVC水平均较治疗前降低,差异有统计学意义(P<0.05);不同严重程度支气管哮喘患者的IL-33、IgE水平比较,表现为轻度<中度<重度,差异有统计学意义(P<0.05);血清IL-33、IgE均与FEV1、FEV1%pred、FEV1/FVC均呈负相关(P<0.05);预后不良的患者血清IL-33和IgE水平高于预后良好的患者,差异有统计学意义(P<0.05)。结论支气管哮喘急性发作并肺部感染患者血清IL-33及IgE水平越高,预示着患者的肺功能及预后状况越差。
Objective To analyze the correlation between serum inflammatory factors,immunoglobulin levels and prognosis in patients with acute exacerbation of bronchial asthma complicated with pulmonary infection.Methods A total of 80 patients with acute exacerbation of bronchial asthma complicated with pulmonary infection(observation group)and 80 healthy volunteers(control group)admitted to our hospital from January 2018 to January 2020 were selected as subjects.The patients in the observation group were treated with standardized treatment.The levels of serum interleukin-33(IL-33),immunoglobulin E(IgE)and lung function were compared between the two groups,and the relationship between IL-33,IgE and lung function and prognosis were analyzed.Results The levels of serum IL-33 and IgE in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The levels of IL-33,IgE,FEV1,FEV1%pred and FEV1/FVC in the observation group were lower than those before treatment,and the difference was statistically significant(P<0.05).The levels of IL-33 and IgE in patients with bronchial asthma at different severity levels were compared,showing mild<moderate<severe,and the difference was statistically significant(P<0.05).Serum IL-33 and IgE were negatively correlated with FEV1,FEV1%pred and FEV1/FVC(P<0.05).The serum IL-33 and IgE levels in patients with poor prognosis were higher than those in patients with good prognosis,and the difference was statistically significant(P<0.05).Conclusion The higher the levels of serum IL-33 and IgE in patients with acute exacerbation of bronchial asthma and pulmonary infection,the worse the pulmonary function and prognosis.
