针灸联合推拿对神经根型颈椎病患者颈肩部疼痛和颈椎活动度的影响

Effect of Acupuncture Combined with Massage on Cervical Shoulder Pain and Cervical Mobility in Patients with Cervical Spondylotic Radiculopathy

目的探讨针灸联合推拿对神经根型颈椎病患者颈肩部疼痛和颈椎活动度的影响。方法选取2019年2月-2021年1月在我院接受治疗的60例神经根型颈椎病患者,随机分为对照组和联合组,每组30例。对照组给予推拿治疗,联合组给予针灸联合推拿进行治疗。对比两组患者治疗前后的各项疼痛评分指标[视觉疼痛模拟评分(VAS)、现有疼痛强度(PPI)和疼痛分级指数(PRI)]、颈椎活动度、炎症因子水平[核转录因子-κB(NF-κB)、白细胞介素-8(IL-8)和白细胞介素-10(IL-10)]、椎动脉血流动力学[时间平均流速(TMFV)、收缩期血流速度(PSV)、搏动指数(PI)、阻力指数(RI)]以及临床疗效。结果治疗后,联合组VAS评分、PPI评分、PRI指数、NF-κB、IL-8、PI、RI均低于治疗前和对照组,颈椎活动度、IL-10水平、TMFV、PSV均高于治疗前和对照组,差异具有统计学意义(P<0.05)。联合组总有效率高于对照组(86.67%vs 66.67%),差异具有统计学意义(P<0.05)。结论针灸联合推拿治疗神经根型颈椎病,疗效显著,能有效缓解颈肩部疼痛,改善颈椎活动度,降低患者体内的炎症因子水平。

Objective To investigate the effect of acupuncture and massage on cervical shoulder pain and cervical mobility in patients with cervical spondylotic radiculopathy.Methods A total of 60 patients with cervical spondylotic radiculopathy who were treated in our hospital from February 2019 to January 2021 were randomly divided into control group and combined group,with 30 cases in each group.The control group was treated with massage,and the combined group was treated with acupuncture combined with massage.The pain score indexes[visual analogue scale(VAS),present pain intensity(PPI)and pain rating index(PRI)],cervical activity,inflammatory factor levels[nuclear factor-κB(NF-κB),interleukin-8(IL-8)and interleukin-10(IL-10)],vertebral artery hemodynamics[time mean flow velocity(TMFV),peak systolic velocity(PSV),pulsatility index(PI),resistance index(RI)]and clinical efficacy were compared between the two groups before and after treatment.Results After treatment,VAS score,PPI score,PRI index,NF-κB,IL-8,PI and RI in the combined group were lower than those before treatment and those in the control group,and cervical mobility,IL-10 level,TMFV and PSV were higher than those before treatment and those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of the combined group was higher than that of the control group(86.67%vs 66.67%),and the difference was statistically significant(P<0.05).Conclusion Acupuncture combined with massage in the treatment of cervical spondylotic radiculopathy can effectively relieve neck and shoulder pain,improve cervical mobility and reduce the level of inflammatory factors in patients with cervical spondylotic radiculopathy.
