探讨EMR联合金属钛夹治疗肠息肉的效果

Discussion on the effect of EMR combined with metal titanium clips in the treatment of intestinal polyps

目的 探讨内镜下黏膜切除术(EMR)联合金属钛夹治疗肠息肉的效果。方法 200例肠息肉患者,以随机数字表法分为参照组和研究组,每组100例。两组术前均行结肠镜检查,并实施灌肠治疗,研究组患者采用EMR联合金属钛夹治疗,参照组患者采用EMR治疗。比较两组术中出血率及出血量,术后迟发性出血率及出血量,术前及术后血红蛋白(Hb)、纤维蛋白原(FIB)水平。结果 研究组术中出血率2.00%(2/100)及出血量(12.30±2.43)ml与参照组的4.00%(4/100)、(12.34±2.41)ml比较,差异无统计学意义(χ2=0.687,t=0.019,P>0.05)。研究组术后迟发性出血率3.00%(3/100)及出血量(24.32±5.20)ml均低于参照组的10.00%(10/100)、(34.80±6.05)ml,差异有统计学意义(χ2=4.031、t=2.696,P<0.05)。两组术前Hb、FIB水平比较,差异无统计学意义(P>0.05);术后,两组Hb、FIB水平均低于术前,但研究组高于参照组,差异有统计学意义(P<0.05)。结论 EMR联合金属钛夹治疗肠息肉患者,能够有效降低术后迟发性出血风险,减少术后Hb、FIB水平下降,具有较高临床应用价值。

Objective To discuss the effect of endoscopic mucosal resection(EMR)combined with metal titanium clips in the treatment of intestinal polyps.Methods A total of 200 patients with intestinal polyps were divided into the reference group and the research group by the random numerical table,with 100 cases in each group.Preoperative colonoscopy was performed in both groups,and enemas were administered.Patients in the research group were treated with EMR and metal titanium clips,and patients in the reference group were treated with EMR.Both groups were compared in terms of intraoperative bleeding rate and blood loss,postoperative delayed bleeding rate and blood loss,hemoglobin(Hb)and fibrinogen(FIB)levels before and after surgery.Results The intraoperative bleeding rate 2.00%(2/100)and blood loss(12.30±2.43)ml in the research group were not statistically significant compared with 4.00%(4/100)and(12.34±2.41)ml in the reference group(χ2=0.687,t=0.019,P>0.05).The postoperative delayed bleeding rate 3.00%(3/100)and blood loss(24.32±5.20)ml in the research group were both lower than 10.00%(10/100)and(34.80±6.05)ml in the reference group,and the difference was statistically significant(χ2=4.031,t=2.696,P<0.05).There was no statistically significant difference in Hb and FIB levels between the two groups before surgery(P>0.05).After surgery,Hb and FIB levels of the two groups were lower than those before surgery,and the research group was higher than the reference group.All the differences were statistically significant(P<0.05).Conclusion Combination of EMR and metal titanium clip for patients with intestinal polyps can effectively reduce the risk of postoperative delayed bleeding and decrease the levels of Hb and FIB,which has high clinical application value.
