额叶α波不对称与抑郁症自杀行为关系

The relationship between frontal alpha asymmetry and suicidal behavior in depression

目的通过对比有、无自杀行为抑郁症患者额叶α波不对称(frontal alpha asymmetry,FAA)指标,了解抑郁症患者FAA特征与抑郁症患者自杀行为的关系。方法纳入56例抑郁症患者,根据既往是否实施过自杀行为分为有自杀行为组(16例)、无自杀行为组(40例)。收集被试者的一般资料,使用脑电图检测FAA,评估贝克自杀意念量表(Beck scale for suicide ideation,BSS)。结果有、无自杀行为组抑郁症患者性别、年龄、受教育程度、汉密尔顿抑郁量表评分、汉密尔顿焦虑量表评分差异无统计学意义(P>0.05)。有、无自杀行为组BSS自杀意念评分差异无统计学意义(P>0.05),BSS自杀风险评分差异有统计学意义(43.62±19.81 vs.27.78±21.90,P=0.015)。有、无自杀行为组FP1/FP2位置FAA差异有统计学意义[0.06(-0.05,0.14)vs.0.16(0.06,0.32),P=0.006]。自杀行为与FP1/FP2位置FAA负相关(r=-0.349,P=0.008)。logistic回归分析显示抑郁症患者自杀行为与FP1/FP2位置FAA的关联有统计学意义(OR=0.008,P=0.024)。结论有自杀行为史的抑郁症患者左前额叶皮质活动减弱、右前额叶皮质活动增强。

Objective To compare the frontal alpha asymmetry(FAA)indexes in depression patients with and without suicidal behaviour to understand the relationship between FAA characteristics and suicidal behaviour in depressed patients.Methods Fifty-six patients with depression were divided into two groups according to whether having suicidal behaviour.All patients underwent FAA tests and were assessed by the Hamilton Depression Scale(HAMD-17),Hamilton Anxiety Scale(HAMA),and Beck Scale for Suicide Ideation(BSS).Results There was no significant difference in sex,age,education,HAMD score and HAMA score between the group with and without suicidal behaviour(P>0.05).There was no significant difference in suicidal ideation scores between the groups with and without suicidal behaviour(P>0.05),and there was a significant difference in suicide risk scores(43.62±19.81 vs.27.78±21.90,P=0.015).There was a significant difference in FP1/FP2 FAA between the groups with and without suicidal behavior[median and upper and lower quartiles:0.06(-0.05,0.14)vs.0.16(0.06,0.32),P=0.006],while the differences in FAA of F3/F4 and F7/F8 between the two groups were not significant(P>0.05).Suicidal behaviour was negatively correlated with FAA in the FP1/FP2(r=-0.349,P=0.008).Suicidal ideation and suicide risk scores were not significantly correlated with FAA in the FP1/FP2(P>0.05).The logistic regression analysis showed a significant relationship between suicidal behaviour and FAA of FP1/FP2 in depressed patients(OR=0.008,P=0.024).Conclusion The results have demonstrated that the left prefrontal cortex activity is decreased and the right prefrontal cortex activity is increased in depressed patients with a history of suicidal behavior.
