候选基因多态性与饮食习惯的交互作用对武陵山片区H型高血压患病的影响研究 被引量:2

The association between candidate gene polymorphism together with dietary habits and H-type hypertension in Wuling mountainous area

摘要 目的通过分析武陵山片区人群候选基因MTHFR、CBS、ALDH2易感位点多态性、饮食习惯偏好以及两者的交互作用与H型高血压的发病关联,从而揭示H型高血压的流行病学危险因素。方法2020年10—12月,将875例研究对象分为对照组、单纯高血压组以及单纯高同型半胱氨酸血症(hyperhomocysteine,HHcy)组以及H型高血压组。通过问卷调查获得一般资料,抽取血样本进行生化指标检测,同时对MTHFR、CBS、ALDH2基因对应的易感位点rs1413355、rs6586283以及rs190914158进行测序,分析上述位点多态性与H型高血压的相关性,并采用二分类logistic回归以及线性优势比模型计算饮食习惯与基因分型对H型高血压形成的交互作用。结果武陵山片区人群中rs1413355、rs190914158位点变异均与HHcy有关,但与血压异常无相关性,rs190914158位点变异与胰岛素抵抗以及血脂水平异常有相关性。多因素logistic回归分析显示,在作为独立变量时,易感位点以及饮食习惯均不能导致H型高血压,rs190914158位点突变和腌制饮食偏好之间的交互作用仍然为发病的危险因素,且与线性优势比模型计算结果吻合。结论武陵山片区人群候选基因ALDH2 rs190914158位点变异可能导致包括同型半胱氨酸在内的多种代谢障碍,并且与腌制饮食偏好在促成H型高血压发病过程中存在交互作用。 Objective To decipher the association between the polymorphisms of candidate gene MTHFR,CBS and ALDH2,together with dietary habits and the incidence of H-type hypertension in Wuling mountainous area,to uncover the epidemiological risk factors for H-type hypertension.Methods From October to December,2020,a total of 875 subjects were divided into normal control,hypertension,hyperhomocysteinemia(HHcy)and H-type hypertension group.Demographic information and other study related information were acquired through a questionnaire survey.Meanwhile,blood samples were collected for biochemical assessment,genetic susceptibility was also assessed by sequencing rs1413355,rs6586283 and rs190914158 in MTHFR,CBS and ALDH2 gene to determine the association of polymorphisms with H-type hypertension.Binomial logistic regression and linear dominance ratio model were used to analyze the effect of gene polymorphisms together with dietary habits on the development of H-type hypertension.Results In Wuling Mountainous area,rs1413355 mutation in MTHFRgene and rs190914158 variation in ALDH2 gene were associated with HHcy,but not with elevated blood pressure.In addition,rs190914158 mutation was associated with insulin resistance and dyslipidemia.Although multivariate logistic regression analysis showed that,neither the mutation of polymorphic sites nor dietary habits could directly lead to H-type hypertension when served as an independent variable,the interaction between rs190914158 mutation and eating too much cured foods was still a risk factor for the development of H-type hypertension,which was consistent with the results from linear dominance ratio model.Conclusions The mutation of rs190914158 in candidate gene ALDH2 in Wuling Mountains area may lead to multiple metabolic disorders including abnormal homocysteine level,and its interaction with dietary habits of eating cured food is proved to be a risk factor for the development of H-type hypertension.
