糖尿病患者口腔健康相关生活质量及影响因素分析

Analysis of oral health related quality of life and its influencing factors in patients with diabetes mellitus

[目的]调查糖尿病患者口腔健康相关生活质量(OHRQoL)及其影响因素.[方法]选取2020年7月至2020年10月间就诊的212例2型糖尿病患者,使用一般资料调查表、糖尿病口腔保健知信行调查问卷和中文版口腔健康影响程度量表(OHIP14)进行调查.[结果]212例2型糖尿病患者中OHRQoL尚可者占31.1%,OHIP14得分为(18.63±9.43)分,明显高于其临界值12分.口腔保健知信行得分为(36.34±9.94)分,处于中等偏低水平.糖尿病患者的OHRQoL受年龄、文化程度、口腔保健知信行等多种因素影响(P<0.05);口腔保健知识、态度、行为与OHRQoL具有相关性.多元线性回归分析结果显示,家庭人均月收入、糖尿病病程、口腔保健知信行是OHRQoL的主要影响因素,共解释了OHIP14总分变异的25.4%.[结论]糖尿病患者OHRQoL较差,且受多重因素影响,应采取相应的管理措施以提高糖尿病患者口腔保健知信行水平.

OBJECTIVE To investigate oral health related quality of life(OHRQoL)and its influencing factors in patients with diabetes mellitus.METHODS A total of 212 patients with type 2 diabetes mellitus treated between July 2020 and October 2020 were investigated using the general data questionnaire,diabetes oral health knowledge,belief and practice questionnaire and Chinese version of the oral health impact scale(OHIP-14).RESULTS Among 212 patients with type 2 diabetes mellitus,31.1%had acceptable OHRQoL,and the score of OHIP14 was(18.63±9.43),which was significantly higher than the critical value of 12.The score of oral health knowledge,belief and practice was(36.34±9.94),which was on the medium low level.The OHRQoL of patients with diabetes mellitus was affected by many factors such as age,education level,and oral health knowledge,belief and practice(P<0.05).There was a certain correlation between oral health care knowledge,attitude and behavior with OHRQoL.The results of multiple linear regression analysis showed that the per capita monthly income of the family,the course of diabetes mellitus and the oral health knowledge,belief and practice were the main influencing factors of OHRQoL,which explained 25.4%of the total variation in OHIP-14 scores.CONCLUSION The patients with diabetes mellitus have poor OHRQoL and are affected by multiple factors,and corresponding management measures should be carried out to improve the level of the oral health knowledge,belief and practice in diabetic patients.
