摘要
目的探讨修正性鼻内镜手术治疗复发性鼻窦炎对患者鼻功能变化的客观评价体系。方法研究纳入2018年12月—2020年1月复发性鼻窦炎患者65例。所有患者均给予修正性鼻内镜手术治疗,分别于患者手术开始前和手术治疗6个月后、12个月后行鼻内镜和鼻窦CT检查,参考两者的评分标准评价患者鼻功能,同时分别在患者手术治疗前和手术治疗6个月后、12个月后计算患者的鼻黏膜纤毛运动速率,对比65例患者手术前后鼻功能和鼻黏膜纤毛运动速率的变化;分析鼻内镜、鼻窦CT和纤毛运动速率3项客观检查之间的相关性。结果鼻功能两项客观指标在术后12个月的评分低于术后6个月、术后6个月的评分低于手术前,3个不同阶段对比差异有统计学意义(P<0.05)。在鼻黏膜纤毛运动速率方面,术后12个月的速率快于术后6个月、术后6个月快于手术开始前,3个不同阶段的速率对比差异有统计学意义(P<0.05)。并且65例患者手术开始前、手术后3个月、6个月和12个月鼻功能两项客观指标之间呈正相关性(P<0.05);两者依次与鼻黏膜纤毛运动速率之间均呈负相关性(P<0.05)。结论对于行修正性鼻内镜手术治疗的复发性鼻窦炎患者而言,其鼻内镜、鼻窦CT和纤毛运动速率之间具有良好的相关性,可以此为主体建立评估体系用于评价患者术后鼻功能的变化。
Objective To explore the objective evaluation system of patients'nasal function changes in the treatment of recurrent sinusitis with modified endoscopic sinus surgery.Methods The study included 65 patients with recurrent sinusitis from December 2018 to January 2020.All patients were treated with modified endoscopic sinus surgery.The patients underwent nasal endoscopy and CT scan of the sinuses before the start of the operation,6 months after the operation,and 12 months after the operation.At the same time,the patient’s nasal mucosal cilia movement rate was calculated before and after surgery for 6 months and 12 months,and the changes in nasal function and nasal mucosal cilia movement rate of 65 patients before and after surgery were compared;analyze the correlation between the three objective examinations of nasal endoscopy,sinus CT and ciliary movement rate.Results The scores of the two objective indicators of nasal function at 12 months after surgery were lower than those at 6 months after surgery,and the scores at 6 months after surgery were lower than before surgery,there were statistically significant differences between the three different stages(P<0.05).In terms of nasal mucosal cilia movement rate,the rate at 12 months after surgery was faster than that at 6 months after surgery,and that at 6 months after surgery was faster than before the start of surgery,there were statistically significant differences in the rates of the three different stages(P<0.05).And 65 patients had a positive correlation between the two objective indicators of nasal function before the operation,3 months,6 months and 12 months after the operation(P<0.05);the two were in turn with the nasal mucosal cilia movement rate.All showed negative correlation(P<0.05).Conclusion For patients with recurrent sinusitis treated with modified endoscopic sinus surgery,there is a good correlation between endoscopy,sinus CT and cilia movement rate.This can be used as the main body to establish an evaluation system to evaluate the changes in postoperative nasal function of patients.
