2型糖尿病患者临检过程中应用血清甲状腺激素检测的临床价值研究

摘要 目的评估血清甲状腺激素检测应用2型糖尿病患者临检过程中的使用价值。方法选取该院检验科2018年1月—2019年12月就诊的80例2型糖尿病患者作为观察组,选择同时期该院接诊的80名健康体检者作为对照组,分析两组血清甲状腺激素指标的差异。结果观察组血清游离三碘甲状腺原氨酸水平(4.02±1.33)pmol/L显著小于对照组(5.19±1.28)pmol/L,差异有统计学意义(t=5.669,P<0.001)。年龄越大的患者血清游离三碘甲状腺原氨酸越低(3.68±0.52)pmol/L vs(4.86±0.33)pmol/L,差异有统计学意义(t=12.434,P<0.001)。病程时间越长的患者血清游离三碘甲状腺原氨酸越低(3.70±0.42)pmol/L vs(4.56±0.62)pmol/L,差异有统计学意义(t=6.644,P<0.001)。糖化血红蛋白越高的患者血清游离三碘甲状腺原氨酸越低(3.52±0.81)pmol/L vs(4.19±1.33)pmol/L,差异有统计学意义(t=2.521,P=0.014)。空腹血糖越高的患者血清游离三碘甲状腺原氨酸越低(3.82±0.91)pmol/L vs(4.79±0.82)pmol/L,差异有统计学意义(t=5.000,P<0.001)。结论2型糖尿病患者与健康体检者相比,其血清游离三碘甲状腺原氨酸更低,年龄、病程、空腹血糖和糖化血红蛋白是影响2型糖尿病患者的血清游离三碘甲状腺原氨酸的重要指标。 Objective To evaluate the value of serum thyroid hormone detection in the clinical examination of patients with type 2 diabetes.Methods Selected 80 patients with type 2 diabetes who were treated in the hospital from January 2018 to December 2019 as the observation group,and selected 80 healthy people who received medical examinations in the hospital during the same period as the control group,and analyzeed the two groups of differences in serum thyroid hormone indicators.Results The serum free triiodothyronine level(4.02±1.33)pmol/L of the observation group was significantly lower than that of the control group(5.19±1.28)pmol/L,the difference was statistically significant(t=5.669,P<0.001).The older the patient,the lower the serum free triiodothyronine(3.68±0.52)pmol/L vs(4.86±0.33)pmol/L,the difference was statistically significant(t=12.434,P<0.001).The longer the course of the disease,the lower the serum free triiodothyronine(3.70±0.42)pmol/L vs(4.56±0.62)pmol/L,the difference was statistically significant(t=6.644,P<0.001).Patients with higher glycosylated hemoglobin had lower serum free triiodothyronine(3.52±0.81)pmol/L,(4.19±1.33)pmol/L,the difference was statistically significant(t=2.521,P=0.014).The higher the fasting blood glucose,the lower the serum free triiodothyronine,(3.82±0.91)pmol/L vs(4.79±0.82)pmol/L,the difference was statistically significant(t=5.000,P<0.001).Conclusion Compared with healthy subjects,patients with type 2 diabetes have lower serum free triiodothyronine.Age,course of disease,fasting blood glucose and glycosylated hemoglobin are importent indicators that affect serum free triiodothyronine of patients with type 2 diabetes.
