摘要
目的研究小剂量胰岛素持续滴注治疗小儿糖尿病酮症酸中毒疗效分析。方法纳入2018年10月—2019年10月收治的66例糖尿病酮症酸中毒患儿展开研究,按照随机数字表法分为两组,对照组32例应用常规剂量胰岛素治疗,观察组34例应用小剂量胰岛素治疗。比较两组的治疗效果。结果观察组患儿治疗后的空腹血糖、餐后2 h血糖水平均低于对照组,差异有统计学意义(P<0.05);观察组患儿治疗后的CO2CP、血清硫胺素水平高于对照组,Lac、β-HB水平低于对照组,差异有统计学意义(P<0.05);观察组患儿的临床相关指标优于对照组,临床总有效率高于对照组,差异有统计学意义(P<0.05);观察组患儿中仅出现1例低血糖,并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论对小儿糖尿病酮症酸中毒实施小剂量胰岛素持续滴注的疗效较高,可平稳降糖,促进临床症状改善,低血糖等并发症少,安全有效。
Objective To study the effect of continuous infusion of low-dose insulin in the treatment of children with diabetic ketoacidosis.Methods In this study,66 children with diabetic ketoacidosis admitted from October 2018 to October 2019 were included in the study.They were divided into two groups according to the random number table method.The control group 32 cases were treated with conventional doses of insulin,and the observation group 34 cases were treated with low-dose insulin.Compared the treatment effects of the two groups.Results The fasting blood glucose and 2 h blood glucose levels of the observation group after treatment were lower than those of the control group,the difference was statistically significant(P<0.05);the CO2CP and serum thiamine levels of the observation group after treatment were higher than those of the control group,Lac andβ-HB level was lower than that of the control group,the difference was statistically significant(P<0.05);the clinical indicators of the observation group were better than those of the control group,and the clinical total effective rate was higher than that of the control group,the difference was statistically significant(P<0.05);only one case of hypoglycemia occurred in the observation group.The complication rate lower than that of the control group,the difference was statistically significant(P<0.05).Conclusion Low-dose continuous infusion of insulin for children with diabetic ketoacidosis has a higher effect,can smoothly lower blood glucose,promote improvement of clinical symptoms,and has fewer complications such as hypoglycemia,and is safe and effective.
