POCT检测心肌肌钙蛋白对急诊科急性胸痛的临床价值分析

Clinical Value of Cardiac Troponin Detected by POCT in Acute Chest Pain in Emergency Department

目的:探讨即时检验(POCT)心肌肌钙蛋白I(cTnI)对急诊科急性胸痛患者的临床价值。方法:选择我院急诊科2019年1月至2020年12月期间收治的122例急性胸痛患者,入院后立即抽取外周静脉血2份,分别进行POCT和实验室检测,比较两种方法对cTnI检测结果一致性。结果:122例胸痛患者中,ST段抬高型心肌梗死(STEMI)24例、非ST段抬高型心肌梗死(NSTEMI)34例、不稳定型心绞痛(UA)68例;POCT与实验室对STEMI、NSTEMI、UA患者血清cTnI的检测结果比较差异无统计学意义(P>0.05)实验室检测结果与POCT结果对cTnI检测具有极高一致性(Kappa=0.896、0.912、0.817)。结论:POCT检测心肌cTnI与实验室结果具有较高的一致性,且临床应用方便、快捷,在急性胸痛的临床诊断中具有较高的临床应用价值。

Objective:To investigate the clinical value of cardiac troponin I(cTnI)in patients with acute chest pain in emergency department. Methods:122 patients with acute chest pain admitted to the emergency department of our hospital from January 2019 to December 2020 were selected. Two samples of peripheral venous blood were taken immediately after admission for POCT and laboratory test respectively. The consistency of cTnI test results between the two methods was compared. Results:There are 24 patients with ST elevation myocardial infarction(STEMI),34 non ST elevation myocardial infarction(NSTEMI),68 unstable angina(UA)in 122 chest pain patients. There was no significant difference between POCT and laboratory in the detection of cTnI in serum of STEMI,NSTEMI and UA patients(P>0.05). The two methods have high consistency in cTnI detection(kappa=0.896,0.912,0.817). Conclusion:POCT detection of cardiac c TnI has high consistency with the laboratory results,and the clinical application is convenient and fast,which has high clinical application value in the clinical diagnosis of acute chest pain.
