急性缺血性脑卒中后不同阿替普酶溶栓时间的预后对比分析

Comparison of prognosis of different alteplase thrombolysis time after acute ischemic stroke

目的:分析急性缺血性脑卒中后不同阿替普酶溶栓时间效果以及患者预后情况。方法:选取我院收治的急性缺血性脑卒中患者78例为本实验研究对象,根据阿替普酶溶栓时间窗的不同,将研究对象分为A、B、C三组,A组给药时间<3 h,研究对象23例;B组给药时间在3-5 h,研究对象29例;C组给药时间>5 h,研究对象26例。给予所有患者同样的常规药物治疗,并按照不同时间窗给予各组患者阿替普酶溶栓治疗。对治疗前后三组患者PRDX1、LPA水平以及Barthel、NIHSS评分结果进行统计和比较。结果:治疗后,三组患者PRDX1、LPA水平均有所降低,且A组水平显著低于B组以及C组,Barthel评分,A组显著高于B组以及C组,NIHSS评分A组显著低于B组以及C组,均P<0.05。结论:在急性脑卒中发生早期即进行阿替普酶溶栓治疗效果显著,降低患者PRDX1、LPA水平效果理想,同时可有效提高患者生活自理能力,具有理想预后,具有较高的临床参考价值。

Objective:To analyze the effect of different alteplase thrombolysis time and prognosis of patients after acute ischemic stroke. Methods:Seventy-eight patients with acute ischemic stroke admitted to our hospital were selected as the subjects of this experiment. According to the different thrombolytic time window of alteplase,the subjects were divided into three groups,A,B and C. Group A had less than 3 h administration time,with 23 subjects. Group B was administrated for 3-5 h,and 29 patients were enrolled. In group C,26 patients were treated at >5 h. All patients were given the same routine drug therapy,and patients in each group were given alteplase thrombolytic therapy according to different time Windows. The levels of PRDX1 and LPA,Barthel and NIHSS scores of the three groups before and after treatment were statistically compared. Results:Before treatment,there were no significant differences in PRDX1 and LPA levels,Barthel and NIHSS scores among the three groups(P<0.05). After treatment,PRDX1 and LPA levels in all three groups were reduced,and the level of group A was significantly lower than that of group B and C,Barthel score in group A was significantly higher than that of group B and C,and NIHSS score in group A was significantly lower than that of group B and C,all P<0.05. Conclusion:In the early stage of acute stroke,alteplase thrombolytic therapy has a significant effect,ideal effect in reducing the PRDX1 and LPA levels of patients,and can effectively improve the ability of patients to take care of themselves,with ideal prognosis and high clinical reference value.
