子宫下段连续折叠缝合预防瘢痕子宫产妇剖宫产产后出血的效果观察

Effect of continuous fold suture in lower segment of uterus on prevention of postpartum hemorrhage in cesarean section of scar uterus

目的探讨子宫下段连续折叠缝合预防瘢痕子宫产妇剖宫产产后出血的效果。方法回顾性分析2017年6月至2019年10月于本院行剖宫产术的瘢痕子宫产妇96例的临床资料,将其中采用常规缝合的48例纳入对照组,将采用子宫下段连续折叠缝合的48例纳入观察组。比较两组产妇临床疗效、产后2 h及24 h出血量、产后抑郁及焦虑量表评分、相关并发症的发生率。结果观察组产妇治疗总有效率(97.82%)高于对照组(83.33%),差异有统计学意义(P<0.05);观察组产妇2 h及24 h产后出血量、SDS及SAS量表评分均低于对照组,差异有统计学意义(P<0.05);观察组产妇产后并发症总发生率(14.58%)低于对照组(39.58%),差异有统计学意义(P<0.05)。结论对瘢痕子宫产妇剖宫产后行子宫下段连续折叠缝合可明显减少产后出血量,预防产后出血,改善产妇产后负面情绪,减少相关并发症的发生,值得临床推广。

Objective To explore the effect of continuous fold suture on the lower segment of uterus to prevention of postpartum hemorrhage in cesarean section of scar uterus.Methods The clinical data of 96 cases of scar uterus parturient undergoing cesarean section from June 2017 to October 2019 were analyzed retrospectively,the control group included 48 patients with conventional suture,48 cases with continuous folding suture in the lower uterine segment were included in the observation group.The clinical efficacy,postpartum 2 h and 24 h bleeding volume,postpartum depression and anxiety scale scores,the incidence of related complications between two groups were compared.Results The total effective rate in the observation group(97.82%)was higher than the control group(83.33%),the difference was statistically significant(P<0.05);the scores of 2 h and 24 h postpartum hemorrhage,SDS and SAS scores were lower than those of control group,the difference was statistically significant(P<0.05);the incidence of complications in the observation group(14.58%)was lower than the control group(39.58%),the difference was statistically significant(P<0.05).Conclusion Continuous folding and suturing of the lower segment of the uterus after cesarean section can significantly reduce the amount of postpartum hemorrhage,prevention of postpartum hemorrhage,improving negative post-natal mood,reduce the incidence of related complications,it is worthy of clinical promotion.
