目的探索军队保健干部慢病全程管理的路径和方法,为保健干部长期的健康保障提供思路。方法采用借助军队干部保健信息平台与互联网医院两大信息平台,发掘其在疾病管理中的优势和潜在性能,设计多层次的信息互通和综合利用方法,整合现有医院的就诊和保健功能,满足疾病管理需求。结果双平台的合理使用可以实现军队保健干部慢病管理的全程需要。结论打造基于双平台互通的保健干部慢病管理模式是可行的,对提升慢病管理效能、降低慢病管理风险、打造更完善流畅的慢病管理体系有着较好的促进作用。

Objective To explore the paths and methods of the whole process management of chronic dis⁃eases among military health cadres,so as to provide ideas for the long-term health protection of health cadres.Methods The military cadre healthcare information platform and Internet hospital information platform were used to explore the advantages and potential performance of the platform in disease management,design multi-level in⁃formation exchange and comprehensive utilization methods,integrate the treatment and healthcare functions of exist⁃ing hospitals,and meet the needs of disease management.Results The rational use of dual platforms could meet the whole process needs of chronic disease management of military health cadres.Conclusion It is feasible to build a chronic disease management model for healthcare cadres based on two platforms,which plays a good role in improving the efficiency of chronic disease management,reducing the risk of chronic disease management and building a more perfect and smooth chronic disease management system.
