耳穴贴压在群组化母婴保健泌乳服务中的应用

Application of ear acupoint pressing in centering group care of maternity and infants lactation health care service

目的探讨产后即刻耳穴贴压在群组化母婴保健泌乳服务中的影响,为临床实施提供循证依据。方法选择2019年1月至2020年6月360例在南方医科大学南海医院住院分娩的产妇作为研究对象,随机分为观察组(n=180)和对照组(n=180)。对照组采用常规护理,观察组在常规护理的基础上加上耳穴贴压,在群组化母婴保健泌乳中应用。取穴:主穴取乳腺、内分泌、胸,肝郁气滞型加肝、神门,气血虚弱型加脾、胃。耳穴取双侧,于产后1 h即刻贴压,留置72 h。嘱患者每天按压3~4次,在哺乳前30分钟进行,以达到酸、麻、胀、热感为宜。收集产后8 h、24 h、24~48 h、48~72 h的泌乳量,评估产后第1天、第2天、第3天产妇泌乳效果。结果观察组产妇产后8 h、24 h、24~48 h、48~72 h的泌乳量均明显高于对照组,差异具有统计学意义(P<0.05)。产后第1天、第2天观察组产妇泌乳效果评价比对照组好,差异具有统计学意义(P<0.05)。结论产后即刻耳穴贴压对产妇的早期泌乳和泌乳量促进均具有积极的影响,能促进产妇乳汁分泌,增加泌乳量,提高母乳喂养的成功率。

Objective To explore the impact of ear acupoint pressing immediately after delivery in the centering group care of maternity and infants lactation service of midwives,and to provide evidence-based evidence for clinical implementation.Methods A total of 360 pregnant women who gave birth in Nanhai Hospital,Southern Medical University from January 2019 to June 2020 were selected as the research objects,and they were randomly divided into observation group(n=180)and control group(n=180).In the observation group,ear acupoint pressing was performed on the basis of the routine nursing in the centering group care of maternity and infants lactation health care service.Acupoint selection were selected.The main acupoints for breast,endocrine,and chest,liver and qi stagnation type with liver and Shenmen,qi and blood weakness type with spleen and stomach.Instruct patients to press 3~4 times a day,30 minutes before breastfeeding,in order to achieve acidity,numbness,swelling,and heat.Auricular points were taken from both sides,pressed 1 h postpartum,and left for 72 h.To collect the lactation volume at 8 h,24 h,24~48 h,48~72h after delivery,and evaluate the maternal lactation effect on the first,second,and third day after delivery.Results The postpartum lactation volume of the observation group at 8 h,24 h,24~48 h and 48~72 h was significantly higher than that of the control group,with statistically significant difference(P<0.05).The lactation effect of puerperant on day 1 and day 2 after delivery was significantly better than that of the control group,with statistically significant difference(P<0.05).Conclusions Ear acupoint pressing immediately after childbirth has a positive effect on early lactation and lactation volume of the primipara.It can promote milk secretion,increase lactation volume,and effectively improve the success rate of breastfeeding.
