经皮微创撬拨复位内固定手术与“L”型切口手术治疗跟骨骨折的临床效果比较

Comparison of Clinical Effect of Percutaneous Minimally Invasive Prying Reduction and Internal Fixation and“L”Incision in the Treatment of Calcaneal Fracture

目的:比较经皮微创撬拨复位内固定手术与"L"型切口手术治疗跟骨骨折的临床效果。方法:选取2015年1月-2019年12月本院收治的62例跟骨骨折患者作为研究对象,按照随机数字表法分为对照组(n=31)和研究组(n=31)。对照组行"L"型切口手术治疗,研究组行经皮微创撬拨复位内固定手术治疗。比较两组AOFAS评分、踝关节功能评分、VAS评分、Bhler角、Gissane角、跟骨高度、跟骨宽度及骨折恢复优良率情况。结果:研究组术后3个月AOFAS评分及踝关节功能评分高于对照组,研究组术后3 d VAS评分低于对照组(P<0.05)。术后3 d,研究组Bhler角、Gissane角及跟骨高度均大于对照组,跟骨宽度低于对照组(P<0.05)。研究组并发症发生率低于对照组(P<0.05)。研究组骨折恢复优良率高于对照组(P<0.05)。结论:与"L"型切口手术治疗相比较,应用经皮微创撬拨复位内固定手术治疗跟骨骨折患者效果更为显著,患者术后跟骨骨折恢复情况较好,可以有效改善踝关节功能,缓解疼痛,有利于进一步促进其关节面复位,加快康复,值得大力推广。

Objective:To compare the clinical effect of percutaneous minimally invasive prying reduction and internal fixation and“L”incision in the treatment of calcaneal fracture.Method:A total of 62 patients with calcaneal fracture admitted to our hospital from January 2015 to December 2019 were selected as the research subjects,and randomly divided into control group(n=31)and study group(n=31).The control group received“L”type incision surgery,and the study group received percutaneous minimally invasive prying reduction and internal fixation.AOFAS score,ankle joint function score,VAS score,Bhler angle,Gissane angle,calcaneal height,calcaneal width and good rate of fracture recovery between the two groups were compared.Result:AOFAS score and ankle function score at 3 months after surgery in the study group were higher than those in the control group,and VAS score at 3 d after surgery in the study group was lower than that in the control group(P<0.05).3 d after surgery,the Bhler angle,Gissane angle and height of calcaneus in the study group were higher than those in the control group,and the width of calcaneus was lower than that in the control group(P<0.05).The incidence of complications in study group was lower than that in control group(P<0.05).The excellent rate of fracture recovery in the study group was higher than that in the control group(P<0.05).Conclusion:Compared with“L”type of incision surgical treatment,the effect of of percutaneous minimally invasive prying reduction and internal fixation in the treatment of calcaneal fractures patients is more significant.The postoperative recovery of calcaneal fracture is better,which can effectively improve the ankle function,relieve pain,further promote the reduction of articular surface and accelerate rehabilitation,which is worthy of great promotion.
