摘要
目的:分析血管内皮生长因子(VEGF)、脂质运载蛋白2(LCN2)、超敏C反应蛋白(hs-CRP)在老年冠心病患者中的诊断价值,并探讨其与冠状动脉侧支循环形成的相关性。方法:收集2020年6月-2021年5月本院收治的老年冠心病患者480例作为研究组,根据Retrop-Cohen分级分为侧支循环组250例和无侧支循环组230例,侧支循环组又分为循环良好组150例和循环不良组100例;同期收集体检中心健康体检者100例作为对照组。比较VEGF、LCN2、hs-CRP在各组的表达差异,分析其诊断价值,探讨其与冠状动脉侧支循环形成的相关性。结果:研究组VEGF、LCN2、hs-CRP均明显高于对照组(P<0.05);循环良好组VEGF、LCN2均明显高于循环不良组,hs-CRP明显低于循环不良组,差异均有统计学意义(P<0.05);VEGF、LCN2、hs-CRP对冠状动脉侧支循环是否良好的ROC曲线结果显示,联合诊断的AUC为0.861,灵敏度和特异度分别为81.6%、62.7%;Spearman相关性分析显示,VEGF、LCN2、hs-CRP与冠状动脉侧支循环分级均密切相关(P<0.05)。结论:血清VEGF、LCN2、hs-CRP水平与老年冠心病患者冠状动脉侧支循环形成密切相关,联合检测有助于病情的诊断和评估。
Objective:To analyze the diagnostic value of vascular endothelial growth factor(VEGF),lipid carrier protein 2(LCN2)and hypersensitive C-reactive protein(hs-CRP)in elderly patients with coronary heart disease,and to explore the correlation between VEGF,LCN2,hs-CRP and the formation of coronary artery collateral circulation.Method:A total of 480 elderly patients with coronary heart disease admitted to our hospital from June 2020 to May 2021 were collected as the study group.According to Retrop-Cohen classification,they were divided intothe collateral circulation group(n=250)and the non-collateral circulation group(n=230),the collateral circulation group were further divided into the good circulation group(n=150)and the bad circulation group(n=100).At the same time,100 healthy subjects from the physical examination center were collected as the control group.The expression differences of VEGF,LCN2 and hs-CRP among the groups were compared,the diagnostic value was analyzed,and the correlation between VEGF,LCN2,hs-CRP and the formation of coronary artery collateral circulation were explored.Result:The levels of VEGF,LCN2 and hs-CRP in the study group were higher than those in the control group(P<0.05).The levels of VEGF and LCN2 in the good circulation group were significantly higher than those in the poor circulation group,and the level of hs-CRP was significantly lower than that in the poor circulation group,the differences were statistically significant(P<0.05).ROC curve results of whether VEGF,LCN2 and hs-CRP were good for coronary collateral circulation showed that the AUC of combined diagnosis was 0.861,and the sensitivity and specificity were 81.6%and 62.7%respectively.Spearman correlation showed that VEGF,LCN2 and hs-CRP were closely correlated with the grade of coronary collateral circulation(P<0.05).Conclusion:Serum VEGF,LCN2 and hs-CRP levels are closely related to the formation of coronary artery collateral circulation in elderly patients with coronary heart disease,and the combined detection is helpful for the diagnosis and evaluation of the disease.
