血清胱抑素C、25羟维生素D及脂蛋白相关磷脂酶A2与2型糖尿病患者颈动脉内膜中层厚度的相关性分析

摘要 目的:探究血清胱抑素C(Cys C)、25羟维生素D[25(OH)D]及脂蛋白相关磷脂酶A2(Lp-PLA2)与2型糖尿病患者颈动脉内膜中层厚度(IMT)的相关性。方法:回顾性选取2017年2月-2019年2月本院收治的2型糖尿病患者167例作为试验组,根据IMT分为IMT增厚组(IMT≥0.9 mm,n=85)和IMT正常组(IMT<0.9 mm,n=82),另选同期体检健康者80例作为对照组。检测所有受试者血清Cys C、25(OH)D及Lp-PLA2水平,分析2型糖尿病患者颈动脉IMT与血清Cys C、25(OH)D及Lp-PLA2的相关性。结果:试验组患者血清Cys C(2.46±0.45)mg/L、Lp-PLA2(178.43±23.15)μg/L,均显著高于对照组的(0.85±0.21)mg/L、(58.27±7.19)μg/L,试验组的25(OH)D为(24.12±3.65)μg/L,显著低于对照组的(58.47±7.95)μg/L,差异均有统计学意义(P<0.05)。IMT增厚组患者血清Cys C(3.07±0.47)mg/L、Lp-PLA2(263.56±26.35)μg/L,均显著高于IMT正常组的(1.83±0.37)mg/L、(90.18±13.07)μg/L,IMT增厚组的25(OH)D为(15.41±3.36)μg/L,显著低于IMT正常组(33.15±4.63)μg/L,差异均有统计学意义(P<0.05)。血清Cys C、Lp-PLA2与颈动脉IMT均呈正相关(r=0.714、0.681,P<0.001),25(OH)D与颈动脉IMT呈负相关(r=-0.339,P<0.001)。结论:2型糖尿病患者的血清Cys C、25(OH)D及Lp-PLA2表达水平与颈动脉IMT具有相关性。 Objective:To explore the relationship between serum cystatin C(Cys C),25 hydroxyvitamin D[25(OH)D]and lipoprotein-associated phospholipase A2(Lp-PLA2)and carotid intima-media thickness(IMT)in patients with type 2 diabetes.Method:A total of 167 type 2 diabetes patients admitted to our hospital from February 2017 to February 2019 were retrospectively selected as experimental group.According to IMT,they were divided into IMT thickening group(IMT≥0.9 mm,n=85)and normal IMT group(IMT<0.9 mm,n=82),and 80 healthy subjects during the same period were selected as control group.Serum levels of Cys C,25(OH)D and Lp-PLA2 were detected in all subjects,and the correlation between carotid IMT and serum Cys C,25(OH)D and Lp-PLA2 in patients with type 2 diabetes mellitus was analyzed.Result:Serum Cys C(2.46±0.45)mg/L and Lp-PLA2(178.43±23.15)μg/L in experimental group were significantly higher than(0.85±0.21)mg/L and(58.27±7.19)μg/L in control group,25(OH)D in experimental group was(24.12±3.65)μg/L,which was significantly lower than(58.47±7.95)μg/L in control group,the differences were statistically significant(P<0.05).Serum Cys C(3.07±0.47)mg/L and Lp-PLA2(263.56±26.35)μg/L in IMT thickening group were significantly higher than(1.83±0.37)mg/L and(90.18±13.07)μg/L in normal IMT group,25(OH)D in IMT thickening group was(15.41±3.36)μg/L,which was significantly lower than(33.15±4.63)μg/L in IMT normal group,the differences were statistically significant(P<0.05).Serum Cys C and Lp-PLA2 were positively correlated with carotid IMT(r=0.714,0.681,P<0.001),25(OH)D was negatively correlated with carotid IMT(r=-0.339,P<0.001).Conclusion:The expression levels of serum Cys C,25(OH)D and Lp-PLA2 are correlated with carotid IMT in patients with type 2 diabetes mellitus.
