肩胛骨体部骨折微创钢板内固定技术外侧柱内固定的临床研究

摘要 目的研究肩胛骨体部骨折微创钢板内固定(minimal invasive plating osteosythsis,MIPO)技术外侧柱内固定的手术技巧及临床疗效。方法选取我院自2005年3月至2019年3月收治的肩胛骨体部骨折患者28例,其中男16例,女12例;年龄21~68岁,平均(34.5±8.4)岁。其中左侧17例,右侧11例。车祸伤20例,摔伤或高处坠落伤8例。合并肋骨骨折5例,肱骨近端骨折1例,锁骨骨折3例。28例患者均采用MIPO技术外侧柱内固定治疗,术后6个月随访肩关节Constant-Murley功能评分。结果术后随访时间为6~36个月,平均(29.4±7.3)个月。复查X线片显示骨折均顺利愈合,愈合时间8~13周,平均(9.3±1.8)周。术后6个月时参照肩关节功能Constant-Murley评分,结果89~99分,平均(96.6±3.4)分。其中优11例,良15例,可2例,优良率92.9%。术后无并发症。结论肩胛骨体部骨折MIPO技术外侧柱内固定具有手术切口小、时间短、费用低等优点,是一种治疗肩胛骨体部骨折良好可行的方法。 Objective To study the operative technique and clinical effect of minimal invasive plating osteosythsis(MIPO)technique for scapular body fracture lateral column fixation.Methods 28 cases of scapular body fracture in our hospital from March 2005 to March 2019 were selected,including 16 males and 12 females,aged from 21 to 68 years,with an average of(34.5±8.4)years.There were 17 cases on the left and 11 on the right.There were 20 cases of traffic accident injuries and 8 cases of falling injuries.There were 5 cases of rib fracture,1 case of proximal humerus fracture and 3 cases of clavicle fracture.All 28 patients were treated with MIPO technique for lateral column internal fixation for scapular body fracture.Results All patients were followed up for 6~36 months,with an average of(29.4±7.3)months.During follow-up,X-ray showed that all fractures healed,in the time ranging from 8 to 13 weeks,with an average of(9.3±1.8)weeks.In the sixth month after operation,the Constant-Murley score of shoulder function distributed from 89 to 99 points,with an average of(96.6±3.4)points.11 cases were excellent,15 cases were good and 2 cases were fair.The excellent and good rate reached 92.9%.There were no postoperative complications.Conclusion MIPO technique for lateral column internal fixation of scapular body fractures has the advantages of small incision,short operation time and low operation cost,which is a good and feasible method for the treatment of scapular body fracture.
