倒刺线在脊柱侧弯矫形手术中的疗效分析

Analysis on the therapeutic efficacy of barbed suture in scoliosis correction surgery

目的评价倒刺线与普通缝合线在脊柱侧弯手术中的临床疗效。方法选择2014年1月至2020年8月于南昌大学附属人民医院骨科接受脊柱侧弯矫形手术的患者60例,根据腰背筋膜的缝合材料分为普通缝合线组和倒刺线组,每组各30例。普通缝合线组用1-0薇乔(强生公司)间断缝合,倒刺线组(强生公司)使用1-0倒刺线缝合。记录两组患者年龄、性别比、术前Cobb角度、手术缝合时间、术中初次缝合密闭良好例数、出血量、术后1周VAS评分及伤口并发症发生情况。结果两组患者年龄、性别及术前Cobb角比较,差异无统计学意义(P>0.05)。倒刺线组缝合时间为(6.80±0.95)min,明显短于普通缝合线组的(13.75±1.50)min,差异有统计学意义(P<0.05)。倒刺线组初次密闭性评价良好28例,明显少于普通缝合线组的20例,差异有统计学意义(P<0.05)。两组患者出血量、术后1周VAS评分及伤口并发症发生率比较,差异均无统计学意义(P>0.05)。结论脊柱侧弯手术中使用倒刺线缝合腰背筋膜能够明显缩短缝合手术时间,增强其筋膜局部密闭性,提高手术疗效。

Objective To evaluate the clinical efficacy of barbed suture and conventional suture in scoliosis correction surgery.Methods A total of 60 patients undergoing scoliosis correction surgery admitted to the department of orthopedics of Affiliated People′s Hospital of Nanchang University rom January 2014 to August 2020 were selected,and they were divided into the conventional suture group(n=30)and the barbed suture group(n=30)according to the suture materials of lumbodorsal fascia.The conventional suture group was sutured intermittently with 1-0 Weiqiao(Johnson&Johnson Company),while the barbed suture group(Johnson&Johnson Company)was sutured with 1-0 barbed suture.The age,gender ratio,Cobb angle before operation,suture time in operation,the number of cases with good initial sealing evaluation in operation,hemorrhage volume,VAS score after 1 week of operation and wound complications in both groups were recorded.Results There were no statistically significant differences in age,gender and preoperative Cobb angle between the two groups(P>0.05).The suture time was(6.80±0.95)min in the barbed suture group,which was shorter than that of(13.75±1.50)min in the conventional suture group,with significant difference(P<0.05).The number of 28 cases with good initial sealing evaluation in the conventional suture group was less than that of 20 cases in the conventional suture group,with significant difference(P<0.05).There was no statistically significant difference of hemorrhage volume,VAS score after 1 week of operation,the incidence of wound complications between the conventional suture group and the barbed suture group(P>0.05).Conclusion The application of barbed suture of lumbodorsal fascia with barbed suture in scoliosis correction surgery can obviously shorten the suture operation time,enhance the local sealing of fascia and improve the operation efficacy.
