摘要
目的探讨窄带成像(NBI)支气管镜在早期肺癌诊断中的作用。方法回顾性分析50例肺癌疑似患者的临床资料。所有患者均行白光支气管镜(WLB)检查、NBI支气管镜检查及病理检查。观察支气管镜WLB、NBI模式下的黏膜表现;结合病理检查结果。比较WLB与NBI支气管镜检查对早期肺癌的诊断效能;观察NBI支气管镜操作过程中的不良事件发生率。结果病理检查结果显示,50例患者中,41例阳性,9例阴性。WLB、NBI支气管镜检查分别发现黏膜阳性28、42例。NBI支气管镜检查诊断早期肺癌的灵敏度为95.1%,高于WLB检查的58.5%(P<0.001);NBI支气管镜检查诊断早期肺癌的特异度为66.7%,略高于WLB检查的55.6%(P>0.05)。NBI支气管镜检查中,仅25例气管、支气管局部黏膜少量渗血。结论NBI支气管镜检查能够提高早期肺癌的诊断率。
Objective To investigate the role of narrow-band imaging(NBI)bronchoscopy in the diagnosis of early lung cancer.Methods The clinical data of 50 patients with suspected lung cancer were analyzed retrospectively.All patients underwent white light bronchoscopy(WLB),NBI bronchoscopy and pathological examination.The mucosal manifestations under WLB and NBI bronchoscopy mode were observed.Combined with the results of pathological examination,the diagnostic efficacy of WLB and NBI bronchoscopy in early lung cancer was compared,the incidence of adverse events during NBI bronchoscopy was observed.Results The results of pathological examination showed that 41 cases were positive and 9 were negative among the 50 patients.WLB and NBI bronchoscopy showed positive mucosa in 28 and 42 cases respectively.The sensitivity of NBI bronchoscopy in the diagnosis of early lung cancer was 95.1%,which was higher than 58.5%of WLB(P<0.001),the specificity of NBI bronchoscopy in the diagnosis of early lung cancer was 66.7%,which was slightly higher than 55.6%of WLB(P>0.05).In NBI bronchoscopy,only 25 cases had a small amount of bleeding from the local mucosa of trachea and bronchus.Conclusion NBI bronchoscopy can improve the diagnostic rate of early lung cancer.
