摘要
目的探讨岭南地区心力衰竭病人中医证候与发病节气的关系。方法选取2013年—2019年广州中医药大学第一附属医院住院病历管理系统中632例心力衰竭病人,对其中医证候、发病节气等进行回顾性分析,并利用圆形分布统计法分析发病节气高峰点。结果632例病人中,男342例(54.1%),女290例(45.9%);青年(<40岁)33例(5.2%),中年(40~64岁)195例(30.9%),老年(≥65岁)404例(63.9%)。心肾阳虚证(26.0%)与气虚血瘀证(23.0%)为最常见的两个证候;小暑为发病率最高的节气;立秋为总体发病节气高峰点,趋势平缓;白露为阳虚水泛证发病节气高峰点,趋势平缓。结论岭南地区心力衰竭病人中医证候与发病节气有关,其发病率最高节气(小暑)、总体发病节气高峰点(立秋)及阳虚水泛证发病节气高峰点(白露)有内在规律可循,在临床防治中应给予重视。
Objective To explore the relationship between traditional Chinese medicine(TCM)syndromes and onset solar terms in patients with heart failure in Lingnan.Methods A total of 632 caseswith heart failure were retrospectively analyzed in TCM syndromes,solar terms.The circular distribution statistics was used to analyze the peak point of onset solar terms.Results Among the 632 hospitalized patients with heart failure,there were 342(54.1%)males,290(45.9%)females,33(5.2%)young people(<40 years old),195(30.8%)middle-aged people(40~64 years old),and 404(64.0%)elderly people(≥65 years old).Yang deficiency of heart and kidney(26.0%)and Qi deficiency and blood stasis(23.0%)were the two most common syndromes.Minor heat was the highest incidence of solar terms.Autumn begins was the peak point of the overall solar terms,with gentle trend.White dew was the peak point of the solar terms of the edema due to Yang deficiency,with a gentle trend.Conclusion There is a relationship between TCM syndromes and onset solar terms in patients with heart failure in Lingnan.The highest incidence of solar terms(minor heat),the peak point of overall solar terms(autumn begins),and the peak point of solar terms of edema due to Yang deficiency(white dew)have inherent rules to follow,and more attention should be paid in clinical prevention and treatment.
